Controller: add CNTLID, AVSCC, APSTA, NVSCC, ACWU, SGLS fields.
Namespace: add NMIC, RESCAP, EUI64 fields. EUI64 is specifically
interesting, since it can be used to construct an UEFI NVMe device
path for a boot entry.
