tcm_qla2xxx: Add >= 24xx series fabric module for target-core
This patch adds support for tcm_qla2xxx fabric module for target-core using
the new qla_target.c LLD logic. This includes support for explict NodeACLs
via configfs using tcm_qla2xxx_setup_nacl_from_rport() from libfc
struct fc_host->rports, and demo-mode support for virtual LUN=0 access.
This patch also adds support for using tcm_qla2xxx_lport->lport_fcport_map
and ->lport_loopid_map to track struct se_node_acl pointers for
individual 24-bit Port ID and 16-bit Loop ID values for qla_target_template
->find_sess_by_s_id() and ->find_sess_by_loop_id() used in a number of
locations into the primary I/O dispatch logic in qla_target.c LLD code.
The main piece for FC Nexus setup is in tcm_qla2xxx_check_initiator_node_acl(),
which calls tcm_qla2xxx_set_sess_by_[s_id,loop_id]() to setup our
lport->lport_fcport_map and lport_loopid_map pointers respectively, and
register the new nexus with TCM via __transport_register_session().

qla2x00_issue_marker() for handling request/response queue processing
for target mode operation
*) Addition of various qla_tgt_mode_enabled() logic checks in
qla24xx_nvram_config(), qla2x00_initialize_adapter(), qla2x00_rff_id(),
qla2x00_abort_isp(), qla24xx_modify_vp_config(), and
qla2x00_vp_abort_isp().

qla2xxx: Add LLD target-mode infrastructure for >= 24xx series
This patch adds the internal qla_target.[c,h] support pieces for qla2xxx
LLD target mode for 24xx series HW. This code was originally based on
external qla2x00t module based on 8.02.01-k4, and has been refactored to push
the bulk of code into mainline qla2xxx.ko LLD -> qla_target.c.
The implementation uses internal workqueues for I/O context submission
into tcm_qla2xxx code, and includes the following API for external
interaction to allow qla2xxx LDD to function without direct target-core
dependencies:
tcm_fc: Fix fc_exch memory leak in ft_send_resp_status
This patch fixes a bug in tcm_fc where fc_exch memory from fc_exch_mgr->ep_pool
is currently being leaked by ft_send_resp_status() usage. Following current
code in ft_queue_status() response path, using lport->tt.seq_send() needs to be
followed by a lport->tt.exch_done() in order to release fc_exch memory back into
libfc_em kmem_cache.
ft_send_resp_status() code is currently used in pre submit se_cmd ft_send_work()
error exceptions, TM request setup exceptions, and main TM response callback
path in ft_queue_tm_resp(). This bugfix addresses the leak in these cases.

target: Convert se_node_acl->acl_group removal to use ->acl_kref
This patch converts core_tpg_del_initiator_node_acl() shutdown from configfs
context to use se_node_acl->acl_kref and ->acl_free_comp in order to wait for
outstanding fabric callbacks to complete via transport_deregister_session()
callbacks before waking ->acl_free_comp from the last ->acl_kref put.
It also changes core_tpg_del_initiator_node_acl() to setup a local sess_list
with target_get_session() + acl->acl_stop = 1 for active sessions that will
be shutdown, and changes transport_deregister_session_configfs() to check
for ->acl_stop usage.

This patch converts the remaining struct se_portal_group->session_lock
usage to use irqsave+irqrestore to address the following warnings for
hardware target mode interrupt context usage. This change generate
other warnings for current iscsi-target mode still using ->session_lock
with spin_lock_bh, which will need to be converted in a seperate patch.
This patch fixes a free after use in lio_target_make_nodeacl() where
iscsi_node_acl was referenced from the original se_nacl_new allocation,
instead of from core_tpg_add_initiator_node_acl() in the case of dynamic
-> explict NodeACL conversion.

In 'mpi_sata_completion', "pm8001_ha->lock" is first released.
It means lock is taken before, which is true for
the context A, as 'pm8001_ha->lock' is taken in 'pm8001_chip_isr()'
But for context B there is no lock taken before and pm8001_ha->lock
is unlocked in 'mpi_sata_completion()'. This may unlock the lock
taken in context A. Possible racing ??
If 'pm8001_ha->lock' is taken in 'process_oq()' instead of
'pm8001_chip_isr' then the above issue can be avoided.

Static checker is giving following warning:

The code flow is as shown below:
process_oq() --> process_one_iomb --> mpi_sata_completion
In 'mpi_sata_completion'
the first call for 'spin_unlock_irqrestore()' is with flags=0,
which is as good as 'spin_unlock_irq()' ( unconditional interrupt
enabling).
So for better performance 'spin_unlock_irqrestore()' can be replaced
with 'spin_unlock_irq()' and 'spin_lock_irqsave()' can be replaced by
'spin_lock_irq()'.

[SCSI] libsas: don't recover end devices attached to disabled phys
If userspace has decided to disable a phy the kernel should honor that
and not inadvertantly re-enable the phy via error recovery. This is
more straightforward in the sata case where link recovery (via
libata-eh) is separate from sas_task cancelling in libsas-eh. Teach
libsas to accept -ENODEV as a successful response from I_T_nexus_reset
('successful' in terms of not escalating further).
This is a more comprehensive fix then "libsas: don't recover 'gone'
devices in sas_ata_hard_reset()", as it is no longer sata-specific.
aic94xx does check the return value from sas_phy_reset() so if the phy
is disabled we proceed with clearing the I_T_nexus.

libata issues follow up srsts when the controller has a hard time
recording the signature-fis after a reset, or if the link supports port
multipliers. libsas does not support port multipliers and no current

Until all sas_tasks are known to no longer be in-flight this flag gates late
completions from colliding with error handling. However, it must be cleared

libsas ata error handling is already async but this does not help the
scan case. Move initial link recovery out from under host->scan_mutex,
and delay synchronization with eh until after all port probe/recovery
work has been queued.
Device ordering is maintained with scan order by still calling
sas_rphy_add() in order of domain discovery.
Since we now scan the domain list when invoking libata-eh we need to be
careful to check for fully initialized ata ports.

ata devices are always scanned after ssp. Prior to the ata error
handling reworks libsas would tend to scan devices in ascending expander
phy order. Restore this ordering by deferring ssp discovery to a
DISCE_PROBE event, and keep the probe order consistent with the
discovery order, not the placement of sata devices.

[SCSI] libsas: let libata recover links that fail to transmit initial sig-fis
libsas fails to discover all sata devices in the domain. If a device fails
negotiation and does not transmit a signature fis the link needs recovery.
libata already understands how to manage slow to come up links, so treat these
conditions as ata device attach events for the purposes of creating an
ata_port. This allows libata to manage retrying link bring up.
Rediscovery is modified to be careful about checking changes in dev_type. It
looks like libsas leaks old devices if the sas address changes, but that's a
fix for another patch.

Holdover from a patch rework, prior to the addition of SAS_DEV_DESTROY
we were holding a reference while the destruct was pending in case the
domain was torn down before the desctruct event ran. That case is
covered by SAS_DEV_DESTROY, and the sas_put_device() just corrupts freed
memory, or worse frees the memory while another agent holds a reference.

ata_port lifetime in libata follows the host. In libsas it follows the
scsi_target. Once scsi_remove_device() has caused all commands to be
completed it allows scsi_remove_target() to immediately proceed to
freeing the ata_port causing bug reports like:
...so arrange for the ata_port to have the same end of life as the domain
device.

When scrolling forward through the eh list (in a clear_q scenario) it is
possible to encounter commands that won the completion vs eh race. Rather
than sprinkle more "if (!task)" throughout the handler just make a pass

Prior to commit 61aaff49 "isci: filter broadcast change notifications
during SMP phy resets" we borrowed the MVS_DEV_EH approach from the
mvsas driver for preventing ->lldd_I_T_nexus_reset() events during ata
discovery. This hack was protecting against the old ->phy_reset() in
ata_bus_probe(), but since the conversion to the new error handling this
hack is preventing resets from reaching ata devices.

[SCSI] isci: remove bus and reset handlers
Remove ->eh_device_reset_handler() and ->eh_bus_reset_handler() for the
same reason they are not implemented for libata hosts, they cannot be
implemented reliably with ata-eh. ATA error recovery wants to divert
all resets to the eh thread and wait for completion, these handlers may
be invoked from a non-blocking ioctl.
The other path they are called from is libsas-eh, and if we escalate
past I_T_nexus reset we have larger problems i.e. tear down all
in-flight commands in the domain potentially without notification to the
lldd if it has chosen not to implement ->lldd_clear_nexus_port() /
->lldd_clear_nexus_ha().

[SCSI] isci: stop interpreting ->lldd_lu_reset() as an ata soft-reset
Driving resets from libsas-eh is pre-mature as libata will make a
decision about performing a softreset. Currently libata determines
whether to perform a softreset based on ata_eh_followup_srst_needed(),
and none of those conditions apply to isci.
Remove the srst implementation and translate ->lldd_lu_reset() for ata
devices as a request to drive a reset via libata-eh.

[SCSI] isci: fix interpretation of "hard" reset
A hard reset to isci in the direct-attached case is one where the driver
internally manages debouncing the link. In the sas-expander-attached
case a hard reset is one that clears affiliations. The driver should
not be prematurely dropping affiliations at run time, that decision (to
force expander hard resets to ata devices) is left to userspace to
manage. So, arrange for I_T_nexus resets to be sas-link-resets in the
expander-attached case and isci-hard-resets in the direct-attached case.

[SCSI] isci: kill isci_port->status
It only tracks whether the port is stopping in order to gate new devices
being discovered while the port is stopping. However, since the check
and subsequent handling is unlocked there is nothing to stop the port
from going down immediately after the check.
Driver is already prepared to handle devices arriving on stale ports,
and those will be cleaned up by an eventual ->lldd_dev_gone()
notification.

[SCSI] isci: kill iphy->isci_port lookups
This field is a holdover from the OS abstraction conversion. The stable
phy to port lookups are done via iphy->ownining_port under scic_lock.
After this conversion to use port->lldd_port the only volatile lookup is
the initial lookup in isci_port_formed(). After that point any lookup
via a successfully notified domain_device is guaranteed to be valid
until the domain_device is destroyed.
Delete ->start_complete as it is only set once and is set as a
consequence of the port going link up, by definition of getting a port
formed event the port is "ready".
While we are correcting port lookups also move the asd_sas_port table
out from under the isci_port. This is to preclude any temptation to use
container_of() to convert an asd_sas_port to an isci_port, the
association is dynamic and under libsas control.

[SCSI] libsas: don't recover 'gone' devices in sas_ata_hard_reset()
The commands that timeout when a disk is forcibly removed may trigger
libata to attempt recovery of the device. If libsas has decided to
remove the device don't permit ata to continue to issue resets to its
last known phy.
The primary motivation for this patch is hotplug testing by writing 0 to
/sys/class/sas_phy/phyX/enable. Without this check this test leads to
libata issuing a reset and re-enabling the device that wants to be torn
down.

[SCSI] libsas: fix sas_find_local_phy(), take phy references
In the direct-attached case this routine returns the phy on which this
device was first discovered. Which is broken if we want to support
wide-targets, as this phy reference can become stale even though the
port is still active.
In the expander-attached case this routine tries to lookup the phy by
scanning the attached sas addresses of the parent expander, and BUG_ONs
if it can't find it. However since eh and the libsas workqueue run
independently we can still be attempting device recovery via eh after
libsas has recorded the device as detached. This is even easier to hit
now that eh is blocked while device domain rediscovery takes place, and
that libata is fed more timed out commands increasing the chances that
it will try to recover the ata device.
Arrange for dev->phy to always point to a last known good phy, it may be
stale after the port is torn down, but it will catch up for wide port
reconfigurations, and never be NULL.

[SCSI] libsas: don't mark expanders as gone when a child device is removed
Commit 56dd2c06 "[SCSI] libsas: Don't issue commands to devices that
have been hot-removed" marked the parent device of an end-device as gone
when all the phys to the end device have been deleted.
The expander device is still present until its parent is removed. This
is a benign change until the smp_execute_task() path is taught to check
->gone.

[SCSI] libsas: poll for ata device readiness after reset
Use ata_wait_after_reset() to poll for link recovery after a reset.
This combined with sas_ha->eh_mutex prevents expander rediscovery from
probing phys in an intermediate state. Local discovery does not have a
mechanism to filter link status changes during this timeout, so it
remains the responsibility of lldds to prevent premature port teardown.
Although once all lldd's support ->lldd_ata_check_ready() that could be
used as a gate to local port teardown.
The signature fis is re-transmitted when the link comes back so we
should be revalidating the ata device class, but that is left to a future
patch.

This patch changes target_submit_tmr() LUN lookup failures to set response
status TMR_LUN_DOES_NOT_EXIST and invoke TFO->queue_tm_rsp(), instead of
sending CHECK_CONDITION status that can cause problems with Linux FCoE
clients.

target: Fix off-by-seven in target_report_luns
cdb_offset is always equal to offset - 8, so remove that one. More
importantly, the existing code only worked correct if
se_cmd->data_length is a multiple of 8. Pass in a length of, say, 9 and
we will happily overwrite 7 bytes of "unallocated" memory.
Now, afaics this bug is currently harmless, as allocations will
implicitly be padded to multiples of 8 bytes. But depending on such a
fact wouldn't qualify as sound engineering practice.

target: Untangle front-end and back-end meanings of max_sectors attribute
se_dev_attrib.max_sectors currently has two independent meanings:
- It is reported in the block limits VPD page as the maximum transfer
length, ie the largest IO that the front-end (fabric) can handle.
Also the target core doesn't enforce this maximum transfer length.
- It is used to hold the size of the largest IO that the back-end can
handle, so we know when to split SCSI commands into multiple tasks.
Fix this by adding a new se_dev_attrib.fabric_max_sectors to hold the
maximum transfer length, and checking incoming IOs against that limit.

target: Don't set WBUS16 or SYNC bits in INQUIRY response
SPC-4 says about the WBUS16 and SYNC bits:
The meanings of these fields are specific to SPI-5 (see 6.4.3).
For SCSI transport protocols other than the SCSI Parallel
Interface, these fields are reserved.
We don't have a SPI fabric module, so we should never set these bits.
(The comment was misleading, since it only mentioned Sync but the
actual code set WBUS16 too).

target: Set peripheral device type consistently in INQUIRY response
Current code sets the peripheral device type to 0x3f == "not present
unknown" for virtual LUN 0 for standard INQUIRY commands, but leaves it
as 0 == "connected direct access block" for VPD INQUIRY commands. This
is just because the check for LUN 0 only happens in some code paths.
Make our peripheral device type consistent by moving the LUN 0 check
into the common emulate_inquiry() code.

target: Fix up handling of short INQUIRY buffers
If the initiator sends us an INQUIRY command with an allocation length
that's shorter than what we want to return, we're simply supposed to
truncate our response and return what the initiator gave us space for,
without signaling any error. Current target code has various tests that
don't fill out the full response if the buffer is too short and
sometimes return errors incorrectly.
Fix this up by allocating a bounce buffer for INQUIRY responses if we
need to, ie if we have cmd->data_length too small as well as
SCF_PASSTHROUGH_SG_TO_MEM_NOALLOC set in cmd->se_cmd_flags -- for most
fabrics, we always allocate at least a full page, but for tcm_loop we
may have a small buffer coming directly from the SCSI stack.
This lets us delete a lot of cmd->data_length checking, and also makes
our INQUIRY handling correct per SPC in a lot more cases.

target: Add TMR_ABORT_TASK task management support
This patch adds initial support for TMR_ABORT_TASK ops for se_cmd
descriptors using se_sess->sess_cmd_list and se_cmd->cmd_kref counting.
It will perform an explict abort for all outstanding se_cmd ops based
upon tmr->ref_task_tag that have not been set CMD_T_COMPLETE.
It will cancel se_cmd->work and wait for backing I/O to complete before
attempting to send SAM_STAT_TASK_ABORTED and perform
target_put_sess_cmd() to release the referenced descriptor.
It also adds a CMD_T_ABORTED check into transport_complete_task() to
catch the completion from backend I/O that has been aborted, and
updates transport_wait_for_tasks() to allow CMD_T_ABORTED usage with
core_tmr_abort_task() context.

transport_generic_request_failure() is a wrapper around calling
transport_send_check_condition_and_sense() that is required once
an se_cmd->cmd_kref has been obtained via target_submit_cmd() ->
target_get_sess_cmd().
tcm_qla2xxx currently requires this, and since it's necessary for
other callers using target_submit_cmd() make it exportable now.

target: Cleanup transport_kunmap_data_sg()
This code isn't broken per se, but it's scary to look at! It looks like
in the t_data_nents==1 case we're doing both a kunmap and a vunmap,
what's saving us is that t_data_vmap in this case is 0, so vunmap
doesn't do anything.
Return after kunmap, so the handling of the three cases does not overlap.

This function makes little sense as a separate abstraction as it's deeply
interwinded with the control flow of its only caller. Merged it into
tcm_loop_queuecommand after factoring out a helper to convert the task
attribute representation.

This allows us to use scsilun_to_int without an ugly cast.
Fix up places that use scsilun_to_int on fcp->fc_lun accordingly.
In fc target, this leaves ft_cmd.lun unused, so remove it.
