cleanup the clock API use of the UART driver which is shared among the
MPC512x and the MPC5200 platforms
- get, prepare, and enable the MCLK during port allocation; disable,
  unprepare and put the MCLK upon port release; hold a reference to the
  clock over the period of use; check for and propagate enable errors
- fix a buffer overflow for clock names with two digit PSC index numbers
- stick with the PPC_CLOCK 'psc%d_mclk' name for clock lookup, only
  switch to a fixed string later after device tree based clock lookup
  will have become available

to achieve support for MPC512x which is neutral to MPC5200, the
modification was done as follows
- introduce "clock alloc" and "clock release" routines in addition to
  the previous "clock enable/disable" routine in the psc_ops struct
- make the clock allocation a part of the port request (resource
  allocation), and make clock release a part of the port release, such
  that essential resources get allocated early
- just enable/disable the clock from within the .clock() callback
  without any allocation or preparation as the former implementation
  did, since this routine is called from within the startup and shutdown
  callbacks
- all of the above remains a NOP for the MPC5200 platform (no callbacks
  are provided on that platform)
- implementation note: the clock gets enabled upon allocation already
  just in case the clock is not only required for bitrate generation but
  for register access as well

The only thing we may have from tun device is the fprog, whic contains
the number of filter elements and a pointer to (user-space) memory
where the elements are. The program itself may not be available if the
device is persistent and detached.

There's a small problem with sk-filters on tun devices. Consider
an application doing this sequence of steps:

fd = open("/dev/net/tun");
ioctl(fd, TUNSETIFF, { .ifr_name = "tun0" });
ioctl(fd, TUNATTACHFILTER, &my_filter);
ioctl(fd, TUNSETPERSIST, 1);
close(fd);

At that point the tun0 will remain in the system and will keep in
mind that there should be a socket filter at address '&my_filter'.

If after that we do

fd = open("/dev/net/tun");
ioctl(fd, TUNSETIFF, { .ifr_name = "tun0" });

we most likely receive the -EFAULT error, since tun_attach() would
try to connect the filter back. But (!) if we provide a filter at
address &my_filter, then tun0 will be created and the "new" filter
would be attached, but application may not know about that.

This may create certain problems to anyone using tun-s, but it's
critical problem for c/r -- if we meet a persistent tun device
with a filter in mind, we will not be able to attach to it to dump
its state (flags, owner, address, vnethdr size, etc.).

The proposal is to allow to attach to tun device (with TUNSETIFF)
w/o attaching the filter to the tun-file's socket. After this
attach app may e.g clean the device by dropping the filter, it
doesn't want to have one, or (in case of c/r) get information
about the device with tun ioctls.

tun: Report whether the queue is attached or not

Multiqueue tun devices allow to attach and detach from its queues
while keeping the interface itself set on file.

Knowing this is critical for the checkpoint part of criu project.

tun: Add ability to create tun device with given index

Tun devices cannot be created with ifidex user wants, but it's
required by checkpoint-restore project.

Long time ago such ability was implemented for rtnl_ops-based
interface for creating links (9c7dafbf net: Allow to create links
with given ifindex), but the only API for creating and managing
tuntap devices is ioctl-based and is evolving with adding new ones
(cde8b15f tuntap: add ioctl to attach or detach a file form tuntap
device).

Following that trend, here's how a new ioctl that sets the ifindex
for device, that _will_ be created by TUNSETIFF ioctl looks like.
So those who want a tuntap device with the ifindex N, should open
the tun device, call ioctl(fd, TUNSETIFINDEX, &N), then call TUNSETIFF.
If the index N is busy, then the register_netdev will find this out
and the ioctl would be failed with -EBUSY.

If setifindex is not called, then it will be generated as before.

Result of skb_frag_dma_map() and dma_map_single() wasn't checked.
Added a check and proper handling in case of failure.
Moved the mapping to the beginning of mlx4_en_xmit(), before updating
the ring data structure to make error handling easier.

When hardware gets into error state, must notify user about it.
When QP in error state no traffic will be tx'ed from the attached
tx_ring.

Driver should know how to recover from this unexpected state. I will send later
on the recovery flow, but having the print shouldn't be delayed.

Fix a bug when FC and PFC are enabled/disabled at the same time.
According to ConnectX-3 Programmer Manual these two features are mutial
exclusive.  So make sure when enabling PFC to turn off global FC and
vise versa.  Otherwise it hurts the performance.

Each function driver instance uses the MAC address of the
lowest function belonging to that physical port as a unique
port identifier. This port identifier is read and cached in
driver during probe and provided to user space through
ndo_get_phys_port_id()

Expand the arm64 image header to allow for co-existance with
PE/COFF header required by the EFI stub.  The PE/COFF format
requires the "MZ" header to be at offset 0, and the offset
to the PE/COFF header to be at offset 0x3c.  The image
header is expanded to allow 2 instructions at the beginning
to accommodate a benign intruction at offset 0 that includes
the "MZ" header, a magic number, and the offset to the PE/COFF
header.

Bluetooth: Add SCO connection fallback

When initiating a transparent eSCO connection, make use of T2 settings
at first try. T2 is the recommended settings from HFP 1.6 WideBand
Speech. Upon connection failure, try T1 settings.

When CVSD is requested and eSCO is supported, try to establish eSCO
connection using S3 settings. If it fails, fallback in sequence to S2,
S1, D1, D0 settings.

To know which setting should be used, conn->attempt is used. It
indicates the currently ongoing SCO connection attempt and can be used
as the index for the fallback settings table.

These setting and the fallback order are described in Bluetooth HFP 1.6
specification p. 101.

Synchronous Connection Complete event can return error "Connection
Rejected due to Limited resources (0x10)".
Handling this error is required for SCO connection fallback. This error
happens when the server tried to accept the connection but failed to
negotiate settings.
This error code has been verified experimentally by sending a T2 request
to a T1 only SCO listener.

Bluetooth: Prevent transparent SCO on older devices

Older Bluetooth devices may not support Setup Synchronous Connection or
SCO transparent data. This is indicated by the corresponding LMP feature
bits. It is not possible to know if the adapter support these features
before setting BT_VOICE option since the socket is not bound to an
adapter. An adapter can also be added after the socket is created. The
socket can be bound to an address before adapter is plugged in.

Thus, on a such adapters, if user request BT_VOICE_TRANSPARENT, outgoing
connections fail on connect() and returns -EOPNOTSUPP. Incoming
connections do not fail. However, they should only be allowed depending
on what was specified in Write_Voice_Settings command.

EOPNOTSUPP is choosen because connect() system call is failing after
selecting route but before any connection attempt.

In order to establish a transparent SCO connection, the correct settings
must be specified in the Setup Synchronous Connection request. For that,
a setting field is added to ACL connection data to set up the desired
parameters. The patch also removes usage of hdev->voice_setting in CVSD
connection and makes use of T2 parameters for transparent data.

When an incoming eSCO connection is requested, check the selected voice
setting and reply appropriately. Voice setting should have been
negotiated previously.  For example, in case of HFP, the codec is
negotiated using AT commands on the RFCOMM channel. This patch only
changes replies for socket with deferred setup enabled.

Bluetooth: Add Bluetooth socket voice option

This patch extends the current Bluetooth socket options with BT_VOICE.
This is intended to choose voice data type at runtime. It only applies
to SCO sockets. Incoming connections shall be setup during deferred
setup. Outgoing connections shall be setup before connect(). The desired
setting is stored in the SCO socket info. This patch declares needed
members, modifies getsockopt() and setsockopt().
