blk-throttle: Take blkcg->lock while traversing blkcg->policy_list
blkcg->policy_list is protected by blkcg->lock. Its not rcu protected
list. So even for readers, they need to take blkcg->lock. There are
few functions which were reading the list without taking lock. Fix it.

In case tag depth is reduced, it is max_depth not real_max_depth.
So we should allow a request with tag >= max_depth, but for a

This is because bdev holds on to disk but disk doesn't pin the
associated queue. If a SCSI device is removed while the device is
still open, the sdev puts the base reference to the queue on release.
When the bdev is finally released, the associated queue is already
gone along with the bdi and bdev_inode_switch_bdi() ends up
dereferencing already freed bdi.
Even if it were not for this bug, disk not holding onto the associated
queue is very unusual and error-prone.
Fix it by making add_disk() take an extra reference to its queue and
put it on disk_release() and ensuring that disk and its fops owner are
put in that order after all accesses to the disk and queue are
complete.

blk-flush: move the queue kick into
A dm-multipath user reported[1] a problem when trying to boot
a kernel with commit 4853abaae7e4a2af938115ce9071ef8684fb7af4
(block: fix flush machinery for stacking drivers with differring
flush flags) applied. It turns out that an empty flush request
can be sent into blk_insert_flush. When the BUG_ON was fixed
to allow for this, I/O on the underlying device would stall. The
reason is that blk_insert_cloned_request does not kick the queue.
In the aforementioned commit, I had added a special case to
kick the queue if data was sent down but the queue flags did
not require a flush. A better solution is to push the queue
kick up into blk_insert_cloned_request.
This patch, along with a follow-on which fixes the BUG_ON, fixes
the issue reported.

block: Remove the control of complete cpu from bio.
bio originally has the functionality to set the complete cpu, but
it is broken.
Chirstoph said that "This code is unused, and from the all the
discussions lately pretty obviously broken. The only thing keeping
it serves is creating more confusion and possibly more bugs."
And Jens replied with "We can kill bio_set_completion_cpu(). I'm fine
with leaving cpu control to the request based drivers, they are the
only ones that can toggle the setting anyway".
So this patch tries to remove all the work of controling complete cpu
from a bio.

x86: Fix S4 regression
Commit 4b239f458 ("x86-64, mm: Put early page table high") causes a S4
regression since 2.6.39, namely the machine reboots occasionally at S4
resume. It doesn't happen always, overall rate is about 1/20. But,
like other bugs, once when this happens, it continues to happen.
This patch fixes the problem by essentially reverting the memory
assignment in the older way.

When allocating a set of jobs from kc->job_pool, job->master_job must be
set (to point to itself) so that the mempool item gets freed when the
master_job completes.

crypto: ghash - Avoid null pointer dereference if no key is set
The ghash_update function passes a pointer to gf128mul_4k_lle which will
be NULL if ghash_setkey is not called or if the most recent call to
ghash_setkey failed to allocate memory. This causes an oops. Fix this
up by returning an error code in the null case.
This is trivially triggered from unprivileged userspace through the
AF_ALG interface by simply writing to the socket without setting a key.
The ghash_final function has a similar issue, but triggering it requires
a memory allocation failure in ghash_setkey _after_ at least one
successful call to ghash_update.

block: initialize the bounce pool if high memory may be added later
init_emergency_pool() does not create the page pool for bouncing block
requests if the current count of high pages is zero. If high memory
may be added later (either via memory hotplug or a balloon driver in a
virtualized system) then a oops occurs if a request with a high page
need bouncing because the pool does not exist.
So, always create the pool if memory hotplug is enabled and change the
test so it's valid even if all high pages are currently in the balloon
(the balloon drivers adjust totalhigh_pages but not max_pfn).

mm: fix race between mremap and removing migration entry
I don't usually pay much attention to the stale "? " addresses in
stack backtraces, but this lucky report from Pawel Sikora hints that
mremap's move_ptes() has inadequate locking against page migration.
mremap's down_write of mmap_sem, together with i_mmap_mutex or lock,
and pagetable locks, were good enough before page migration (with its
requirement that every migration entry be found) came in, and enough
while migration always held mmap_sem; but not enough nowadays, when
there's memory hotremove and compaction.
The danger is that move_ptes() lets a migration entry dodge around
behind remove_migration_pte()'s back, so it's in the old location when
looking at the new, then in the new location when looking at the old.
Either mremap's move_ptes() must additionally take anon_vma lock(), or
migration's remove_migration_pte() must stop peeking for is_swap_entry()
before it takes pagetable lock.
Consensus chooses the latter: we prefer to add overhead to migration
than to mremapping, which gets used by JVMs and by exec stack setup.

sparc: Add alignment flag to PCI expansion resources
Currently no type of alignment is specified for PCI expansion roms while
parsing the openfirmware tree. This causes calls to pci_map_rom() to fail.
IORESOURCE_SIZEALIGN is the default alignment used for rom resouces in
pci/probe.c, and has been verified to work with various cards on a ultra 10.

r8169: fix driver shutdown WoL regression.
Due to commit 92fc43b4159b518f5baae57301f26d770b0834c9 ("r8169: modify the
flow of the hw reset."), rtl8169_hw_reset stomps during driver shutdown on
RxConfig bits which are needed for WOL on some versions of the hardware.
As these bits were formerly set from the r81{0x, 68}_pll_power_down methods,
factor them out for use in the driver shutdown (rtl_shutdown) handler.
I favored __rtl8169_get_wol() -hardware state indication- over
RTL_FEATURE_WOL as the latter has become a good candidate for removal.

block: fix request_queue lifetime handling by making blk_queue_cleanup() properly shutdown
request_queue is refcounted but actually depdends on lifetime
management from the queue owner - on blk_cleanup_queue(), block layer
expects that there's no request passing through request_queue and no
new one will.
This is fundamentally broken. The queue owner (e.g. SCSI layer)
doesn't have a way to know whether there are other active users before
calling blk_cleanup_queue() and other users (e.g. bsg) don't have any
guarantee that the queue is and would stay valid while it's holding a
reference.
With delay added in blk_queue_bio() before queue_lock is grabbed, the
following oops can be easily triggered when a device is removed with
in-flight IOs.

This happens because blk_queue_cleanup() destroys the queue and
elevator whether IOs are in progress or not and DEAD tests are
sprinkled in the request processing path without proper
synchronization.
Similar problem exists for blk-throtl. On queue cleanup, blk-throtl
is shutdown whether it has requests in it or not. Depending on
timing, it either oopses or throttled bios are lost putting tasks
which are waiting for bio completion into eternal D state.
The way it should work is having the usual clear distinction between
shutdown and release. Shutdown drains all currently pending requests,
marks the queue dead, and performs partial teardown of the now
unnecessary part of the queue. Even after shutdown is complete,
reference holders are still allowed to issue requests to the queue
although they will be immmediately failed. The rest of teardown
happens on release.
This patch makes the following changes to make blk_queue_cleanup()
behave as proper shutdown.
* QUEUE_FLAG_DEAD is now set while holding both q->exit_mutex and
queue_lock.
* Unsynchronized DEAD check in generic_make_request_checks() removed.
This couldn't make any meaningful difference as the queue could die
after the check.
* blk_drain_queue() updated such that it can drain all requests and is
now called during cleanup.
* blk_throtl updated such that it checks DEAD on grabbing queue_lock,
drains all throttled bios during cleanup and free td when queue is
released.

attempt_plug_merge() accesses elevator without holding queue_lock and
may call into ->elevator_bio_merge_fn(). The elvator is guaranteed to
be valid because it's accessed iff the plugged list has requests and
elevator is never exited with live requests, so as long as the
elevator method can deal with unlocked access, this is safe.
Explain the sync rules around attempt_plug_merge() and drop the
unnecessary @tsk parameter.
This patch doesn't introduce any functional change.

Currently get_request[_wait]() allocates request whether queue is dead
or not. This patch makes get_request[_wait]() return NULL if @q is
dead. blk_queue_bio() is updated to fail the submitted bio if request
allocation fails. While at it, add docbook comments for
get_request[_wait]().
Note that the current code has rather unclear (there are spurious DEAD
tests scattered around) assumption that the owner of a queue
guarantees that no request travels block layer if the queue is dead
and this patch in itself doesn't change much; however, this will allow
fixing the broken assumption in the next patch.

block: reorganize throtl_get_tg() and blk_throtl_bio()
blk_throtl_bio() and throtl_get_tg() have rather unusual interface.
* throtl_get_tg() returns pointer to a valid tg or ERR_PTR(-ENODEV),
and drops queue_lock in the latter case. Different locking context
depending on return value is error-prone and DEAD state is scheduled
to be protected by queue_lock anyway. Move DEAD check inside
queue_lock and return valid tg or NULL.
* blk_throtl_bio() indicates return status both with its return value
and in/out param **@bio. The former is used to indicate whether
queue is found to be dead during throtl processing. The latter
whether the bio is throttled.
There's no point in returning DEAD check result from
blk_throtl_bio(). The queue can die after blk_throtl_bio() is
finished but before make_request_fn() grabs queue lock.
Make it take *@bio instead and return boolean result indicating
whether the request is throttled or not.
This patch doesn't cause any visible functional difference.

block: drop unnecessary blk_get/put_queue() in scsi_cmd_ioctl() and blk_get_tg()
blk_get/put_queue() in scsi_cmd_ioctl() and throtl_get_tg() are
completely bogus. The caller must have a reference to the queue on
entry and taking an extra reference doesn't change anything.
For scsi_cmd_ioctl(), the only effect is that it ends up checking
QUEUE_FLAG_DEAD on entry; however, this is bogus as queue can die
right after blk_get_queue(). Dead queue should be and is handled in
request issue path (it's somewhat broken now but that's a separate
problem and doesn't affect this one much).
throtl_get_tg() incorrectly assumes that q is rcu freed. Also, it
doesn't check return value of blk_get_queue(). If the queue is
already dead, it ends up doing an extra put.

blkio_policy_parse_and_set() calls blkio_check_dev_num() to check
whether the given dev_t is valid. blkio_check_dev_num() uses

[media] videodev: fix a NULL pointer dereference in v4l2_device_release()
The change in 8280b66 does not cover the case when v4l2_dev is already
NULL, fix that.
With a Kinect sensor, seen as an USB camera using GSPCA in this context,
a NULL pointer dereference BUG can be triggered by just unplugging the
device after the camera driver has been loaded.

If target_level == 0, current code breaks out of the while-loop if
SUPERPAGE bit is set. We should also break out if PTE is not present.
If we don't do this, KVM calls to iommu_iova_to_phys() will cause
pfn_to_dma_pte() to create mapping for 4KiB pages.

set dmar->iommu_superpage field to the smallest common denominator
of super page sizes supported by all active VT-d engines. Initialize
this field in intel_iommu_domain_init() API so intel_iommu_map() API
will be able to use iommu_superpage field to determine the appropriate
super page size to use.

drm/radeon/kms/atom: fix handling of FB scratch indices
FB scratch indices are dword indices, but we were treating
them as byte indices. As such, we were getting the wrong
FB scratch data for non-0 indices. Fix the indices and
guard the indexing against indices larger than the scratch
allocation.
Fixes memory corruption on some boards if data was written
past the end of the FB scratch array.

tproxy: copy transparent flag when creating a time wait
The transparent socket option setting was not copied to the time wait
socket when an inet socket was being replaced by a time wait socket. This
broke the --transparent option of the socket match and may have caused
that FIN packets belonging to sockets in FIN_WAIT2 or TIME_WAIT state
were being dropped by the packet filter.

bonding: use local function pointer of bond->recv_probe in bond_handle_frame
The bond->recv_probe is called in bond_handle_frame() when
a packet is received, but bond_close() sets it to NULL. So,
a panic occurs when both functions work in parallel.
Why this happen:
After null pointer check of bond->recv_probe, an sk_buff is
duplicated and bond->recv_probe is called in bond_handle_frame.
So, a panic occurs when bond_close() is called between the
check and call of bond->recv_probe.
Patch:
This patch uses a local function pointer of bond->recv_probe
in bond_handle_frame(). So, it can avoid the null pointer
dereference.

tg3: negate USE_PHYLIB flag check
USE_PHYLIB flag in tg3_remove_one() is being checked incorrectly. This
results tg3_phy_fini->phy_disconnect is never called and when tg3 module
is removed.
In my case this resulted in panics in phy_state_machine calling function
phydev->adjust_link.
So correct this check.

bluetooth: Properly clone LSM attributes to newly created child connections
The Bluetooth stack has internal connection handlers for all of the various
Bluetooth protocols, and unfortunately, they are currently lacking the LSM
hooks found in the core network stack's connection handlers. I say
unfortunately, because this can cause problems for users who have have an
LSM enabled and are using certain Bluetooth devices. See one problem
report below:
In order to keep things simple at this point in time, this patch fixes the
problem by cloning the parent socket's LSM attributes to the newly created
child socket. If we decide we need a more elaborate LSM marking mechanism
for Bluetooth (I somewhat doubt this) we can always revisit this decision
in the future.

Need to cleanup bridge device timers and ports when being bridge
device is being removed via netlink.
This fixes the problem of observed when doing:
ip link add br0 type bridge
ip link set dev eth1 master br0
ip link set br0 up
ip link del br0
which would cause br0 to hang in unregister_netdev because
of leftover reference count.

Where the first one is enabling a CLOCK_PROCESS_CPUTIME_ID timer, and
the second one is keeping up-to-date.
This problem was introduced by e8abccb7193 ("posix-cpu-timers: Cure
SMP accounting oddities").
Cure the problem by removing the cputimer->lock and rq->lock nesting,
this leaves concurrent enablers doing duplicate work, but the time
wasted should be on the same order otherwise wasted spinning on the
lock and the greater-than assignment filter should ensure we preserve
monotonicity.

ALSA: HDA: conexant support for Lenovo T520/W520
This is patch for Conexant codec of Intel HDA driver, adding new quirk
for Lenovo Thinkpad T520 and W520. Conexant autodetection works fine for
T520 (similar subsystem ID is used also in W520 model) and detects more
mixer features compared to generic (fallback) Lenovo quirk with
hardcoded options in Conexant codec.
Patch was activelly tested with Linux 3.0.4, 3.0.6 and 3.0.7 without any
problems.

The previous fix for the position-buffer check gives yet another
regression on a Dell laptop. The safest fix right now is to add a

x25: Prevent skb overreads when checking call user data
x25_find_listener does not check that the amount of call user data given
in the skb is big enough in per-socket comparisons, hence buffer
overreads may occur. Fix this by adding a check.

x25: Handle undersized/fragmented skbs
There are multiple locations in the X.25 packet layer where a skb is
assumed to be of at least a certain size and that all its data is
currently available at skb->data. These assumptions are not checked,
hence buffer overreads may occur. Use pskb_may_pull to check these
minimal size assumptions and ensure that data is available at skb->data
when necessary, as well as use skb_copy_bits where needed.

x25: Validate incoming call user data lengths
X.25 call user data is being copied in its entirety from incoming messages
without consideration to the size of the destination buffers, leading to
possible buffer overflows. Validate incoming call user data lengths before
these copies are performed.
It appears this issue was noticed some time ago, however nothing seemed to

udplite: fast-path computation of checksum coverage
Commit 903ab86d195cca295379699299c5fc10beba31c7 of 1 March this year ("udp: Add
lockless transmit path") introduced a new fast TX path that broke the checksum
coverage computation of UDP-lite, which so far depended on up->len (only set
if the socket is locked and 0 in the fast path).
Fixed by providing both fast- and slow-path computation of checksum coverage.
The latter can be removed when UDP(-lite)v6 also uses a lockless transmit path.

Avoid using variable-length arrays in kernel/sys.c
The size is always valid, but variable-length arrays generate worse code
for no good reason (unless the function happens to be inlined and the
compiler sees the length for the simple constant it is).
Also, there seems to be some code generation problem on POWER, where
Henrik Bakken reports that register r28 can get corrupted under some
subtle circumstances (interrupt happening at the wrong time?). That all
indicates some seriously broken compiler issues, but since variable
length arrays are bad regardless, there's little point in trying to
chase it down.
"Just don't do that, then".

loop: remove the incorrect write_begin/write_end shortcut
Currently the loop device tries to call directly into write_begin/write_end
instead of going through ->write if it can. This is a fairly nasty shortcut
as write_begin and write_end are only callbacks for the generic write code
and expect to be called with filesystem specific locks held.
This code currently causes various issues for clustered filesystems as it
doesn't take the required cluster locks, and it also causes issues for XFS
as it doesn't properly lock against the swapext ioctl as called by the
defragmentation tools. This in case causes data corruption if
defragmentation hits a busy loop device in the wrong time window, as
reported by RH QA.
The reason why we have this shortcut is that it saves a data copy when
doing a transformation on the loop device, which is the technical term
for using cryptoloop (or an XOR transformation). Given that cryptoloop
has been deprecated in favour of dm-crypt my opinion is that we should
simply drop this shortcut instead of finding complicated ways to to
introduce a formal interface for this shortcut.

Per the text in Documentation/SubmitChecklist as below, we should
explicitly have header linux/errno.h in localtimer.h for ENXIO
reference.
1: If you use a facility then #include the file that defines/declares
that facility. Don't depend on other header files pulling in ones
that you use.
Otherwise, we may run into some compiling error like the following one,
if any file includes localtimer.h without CONFIG_LOCAL_TIMERS defined.

intel-iommu: Export a flag indicating that the IOMMU is used for iGFX.
We really don't want this to work in the general case; device drivers
*shouldn't* care whether they are behind an IOMMU or not. But the
integrated graphics is a special case, because the IOMMU and the GTT are
all kind of smashed into one and generally horrifically buggy, so it's
reasonable for the graphics driver to want to know when the IOMMU is
active for the graphics hardware.

intel-iommu: Workaround IOTLB hang on Ironlake GPU
To work around a hardware issue, we have to submit IOTLB flushes while
the graphics engine is idle. The graphics driver will (we hope) go to
great lengths to ensure that it gets that right on the affected
chipset(s)... so let's not screw it over by deferring the unmap and
doing it later. That wouldn't be very helpful.

The patch titled: "xen/blkback: Fix the inhibition to map pages
when discarding sector ranges." had the right idea except that
it used the wrong comparison operator. It had == instead of !=.
This fixes the bug where all (except discard) operations would
have been ignored.
