PIO and MMIO are separate address spaces, but
ioeventfd registration code mistakenly detected
two eventfds as duplicate if they use the same address,
even if one is PIO and another one MMIO.

ccw_io_helper neglected to reset vcdev->err after a new channel
program had been successfully started, resulting in stale errors
delivered after one I/O failed. Reset the error after a new
channel program has been successfully started with no old I/O
pending.

pmu: prepare for migration support
In order to migrate the PMU state correctly, we need to restore the
values of MSR_CORE_PERF_GLOBAL_STATUS (a read-only register) and
MSR_CORE_PERF_GLOBAL_OVF_CTRL (which has side effects when written).
We also need to write the full 40-bit value of the performance counter,
which would only be possible with a v3 architectural PMU's full-width
counter MSRs.
To distinguish host-initiated writes from the guest's, pass the
full struct msr_data to kvm_pmu_set_msr.

KVM: s390: Remove the sanity checks for kvm memory slot
To model the standby memory with memory_region_add_subregion
and friends, the guest would have one or more regions of ram.
Remove the check allowing only one memory slot and the check
requiring the real address of memory slot starts at zero.

KVM: s390: Change the virtual memory mapping location for virtio devices
The current location for mapping virtio devices does not take
into consideration the standby memory. This causes the failure
of mapping standby memory since the location for the mapping is
already taken by the virtio devices. To fix the problem, we move
the location to beyond the end of standby memory.

KVM: s390: fix and enforce return code handling for irq injections
kvm_s390_inject_program_int() and friends may fail if no memory is available.
This must be reported to the calling functions, so that this gets passed
down to user space which should fix the situation.
Alternatively we end up with guest state corruption.
So fix this and enforce return value checking by adding a __must_check
annotation to all of these function prototypes.

KVM: s390: make if statements in lpsw/lpswe handlers readable
Being unable to parse the 5- and 8-line if statements I had to split them
to be able to make any sense of them and verify that they match the
architecture.
So change the code since I guess that other people will also have a hard
time parsing such long conditional statements with line breaks.
Introduce a common is_valid_psw() function which does all the checks needed.
In case of lpsw (64 bit psw -> 128 bit psw conversion) it will do some not
needed additional checks, since a couple of bits can't be set anyway, but
that doesn't hurt.

KVM: s390: fix psw conversion in lpsw handler
When converting a 64 bit psw to a 128 bit psw the addressing mode bit of
the "addr" part of the 64 bit psw must be moved to the basic addressing
mode bit of the "mask" part of the 128 bit psw.
In addition the addressing mode bit must be cleared when moved to the "addr"
part of the 128 bit psw.
Otherwise an invalid psw would be generated if the orginal psw was in the
31 bit addressing mode.

When checking for validity the lpsw/lpswe handler check that only
the lower 20 bits instead of 24 bits have a non-zero value.
There handling valid psws as invalid ones.

KVM: s390: Dont do a gmap update on minor memslot changes
Some memslot updates dont affect the gmap implementation,
e.g. setting/unsetting dirty tracking. Since a gmap update
will cause tlb flushes and segment table invalidations we
want to avoid that.

kvm/ppc/e500: eliminate tlb_refs
Commit 523f0e5421c12610527c620b983b443f329e3a32 ("KVM: PPC: E500:
Explicitly mark shadow maps invalid") began using E500_TLB_VALID
for guest TLB1 entries, and skipping invalidations if it's not set.
However, when E500_TLB_VALID was set for such entries, it was on a
fake local ref, and so the invalidations never happen. gtlb_privs
is documented as being only for guest TLB0, though we already violate
that with E500_TLB_BITMAP.
Now that we have MMU notifiers, and thus don't need to actually
retain a reference to the mapped pages, get rid of tlb_refs, and
use gtlb_privs for E500_TLB_VALID in TLB1.
Since we can have more than one host TLB entry for a given tlbe_ref,
be careful not to clear existing flags that are relevant to other
host TLB entries when preparing a new host TLB entry.

kvm/ppc/e500: g2h_tlb1_map: clear old bit before setting new bit
It's possible that we're using the same host TLB1 slot to map (a
presumably different portion of) the same guest TLB1 entry. Clear
the bit in the map before setting it, so that if the esels are the same
the bit will remain set.

Add one to esel values in h2g_tlb1_rmap, so that "no mapping" can be
distinguished from "esel 0". Note that we're not saved by the fact
that host esel 0 is reserved for non-KVM use, because KVM host esel
numbering is not the raw host numbering (see to_htlb1_esel).

KVM: PPC: Added one_reg interface for timer registers
If userspace wants to change some specific bits of TSR
(timer status register) then it uses GET/SET_SREGS ioctl interface.
So the steps will be:
i) user-space will make get ioctl,
ii) change TSR in userspace
iii) then make set ioctl.
It can happen that TSR gets changed by kernel after step i) and
before step iii).
To avoid this we have added below one_reg ioctls for oring and clearing
specific bits in TSR. This patch adds one registerface for:
1) setting specific bit in TSR (timer status register)
2) clearing specific bit in TSR (timer status register)
3) setting/getting the TCR register. There are cases where we want to only
change TCR and not TSR. Although we can uses SREGS without
KVM_SREGS_E_UPDATE_TSR flag but I think one reg is better. I am open
if someone feels we should use SREGS only here.

The current name "kvm_mmu_free_some_pages" should be used for something
that actually frees some shadow pages, as we expect from the name, but
what the function is doing is to make some, KVM_MIN_FREE_MMU_PAGES,
shadow pages available: it does nothing when there are enough.
This patch changes the name to reflect this meaning better; while doing
this renaming, the code in the wrapper function is inlined into the main
body since the whole function will be inlined into the only caller now.

KVM: MMU: Move kvm_mmu_free_some_pages() into kvm_mmu_alloc_page()
What this function is doing is to ensure that the number of shadow pages
does not exceed the maximum limit stored in n_max_mmu_pages: so this is
placed at every code path that can reach kvm_mmu_alloc_page().
Although it might have some sense to spread this function in each such
code path when it could be called before taking mmu_lock, the rule was
changed not to do so.
Taking this background into account, this patch moves it into
kvm_mmu_alloc_page() and simplifies the code.
Note: the unlikely hint in kvm_mmu_free_some_pages() guarantees that the
overhead of this function is almost zero except when we actually need to
allocate some shadow pages, so we do not need to care about calling it
multiple times in one path by doing kvm_mmu_get_page() a few times.
Without that commit, I can not boot my G5, thus I can't run automated tests on it against my queue.

The CS base was initialized to 0 on VMX (wrong, but usually overridden

PCI: Use ROM images from firmware only if no other ROM source available
Mantas Mikulėnas reported that his graphics hardware failed to
initialise after commit f9a37be0f02a ("x86: Use PCI setup data").
The aim of this commit was to ensure that ROM images were available on
some Apple systems that don't expose the GPU ROM via any other source.
In this case, UEFI appears to have provided a broken ROM image that we
were using even though there was a perfectly valid ROM available via
other sources. The simplest way to handle this seems to be to just
re-order pci_map_rom() and leave any firmare-supplied ROM to last.

sparc: delete "if !ULTRA_HAS_POPULATION_COUNT"
Commit 2d78d4beb64eb07d50665432867971c481192ebf ("[PATCH] bitops:
sparc64: use generic bitops") made the default of GENERIC_HWEIGHT depend
on !ULTRA_HAS_POPULATION_COUNT. But since there's no Kconfig symbol with
that name, this always evaluates to true. Delete this dependency.

KVM: Fix bounds checking in ioapic indirect register reads (CVE-2013-1798)
If the guest specifies a IOAPIC_REG_SELECT with an invalid value and follows
that with a read of the IOAPIC_REG_WINDOW KVM does not properly validate
that request. ioapic_read_indirect contains an
ASSERT(redir_index < IOAPIC_NUM_PINS), but the ASSERT has no effect in
non-debug builds. In recent kernels this allows a guest to cause a kernel
oops by reading invalid memory. In older kernels (pre-3.3) this allows a
guest to read from large ranges of host memory.
Tested: tested against apic unit tests.

There is a potential use after free issue with the handling of
MSR_KVM_SYSTEM_TIME. If the guest specifies a GPA in a movable or removable
memory such as frame buffers then KVM might continue to write to that
address even after it's removed via KVM_SET_USER_MEMORY_REGION. KVM pins
the page in memory so it's unlikely to cause an issue, but if the user
space component re-purposes the memory previously used for the guest, then
the guest will be able to corrupt that memory.

KVM: x86: fix for buffer overflow in handling of MSR_KVM_SYSTEM_TIME (CVE-2013-1796)
If the guest sets the GPA of the time_page so that the request to update the
time straddles a page then KVM will write onto an incorrect page. The
write is done byusing kmap atomic to get a pointer to the page for the time
structure and then performing a memcpy to that page starting at an offset
that the guest controls. Well behaved guests always provide a 32-byte aligned
address, however a malicious guest could use this to corrupt host kernel
memory.

The Kconfig entry for DEBUG_ERRORS is a verbatim copy of the former arm
entry for that symbol. It got removed in v2.6.39 because it wasn't
actually used anywhere. There are still no users of DEBUG_ERRORS so
remove this entry too.

inet: limit length of fragment queue hash table bucket lists
This patch introduces a constant limit of the fragment queue hash
table bucket list lengths. Currently the limit 128 is choosen somewhat
arbitrary and just ensures that we can fill up the fragment cache with
empty packets up to the default ip_frag_high_thresh limits. It should
just protect from list iteration eating considerable amounts of cpu.
If we reach the maximum length in one hash bucket a warning is printed.
This is implemented on the caller side of inet_frag_find to distinguish
between the different users of inet_fragment.c.
I dropped the out of memory warning in the ipv4 fragment lookup path,
because we already get a warning by the slab allocator.

Give a bad return code when specifying a router setting that is either

KVM: allow host header to be included even for !CONFIG_KVM
The new context tracking subsystem unconditionally includes kvm_host.h
headers for the guest enter/exit macros. This causes a compile
failure when KVM is not enabled.
Fix by adding an IS_ENABLED(CONFIG_KVM) check to kvm_host so it can
be included/compiled even when KVM is not enabled.

hwmon: (lm75.h) Update header inclusion
File lm75.h used to include <linux/hwmon.h> for SENSORS_LIMIT() but

MAINTAINERS: Remove Mark M. Hoffman
Mark M. Hoffman stopped working on the Linux kernel several years
ago, so he should no longer be listed as a driver maintainer. I'm not
even sure if his e-mail address still works.
I can take over 3 drivers he was responsible for, the 4th one will
fall down to the subsystem maintainer.
Also give Mark credit for all the good work he did.

xfs: ensure we capture IO errors correctly
Failed buffer readahead can leave the buffer in the cache marked
with an error. Most callers that then issue a subsequent read on the
buffer do not zero the b_error field out, and so we may incorectly
detect an error during IO completion due to the stale error value
left on the buffer.
Avoid this problem by zeroing the error before IO submission. This
ensures that the only IO errors that are detected those captured
from are those captured from bio submission or completion.

tcp: dont handle MTU reduction on LISTEN socket
When an ICMP ICMP_FRAG_NEEDED (or ICMPV6_PKT_TOOBIG) message finds a
LISTEN socket, and this socket is currently owned by the user, we
set TCP_MTU_REDUCED_DEFERRED flag in listener tsq_flags.
This is bad because if we clone the parent before it had a chance to
clear the flag, the child inherits the tsq_flags value, and next
tcp_release_cb() on the child will decrement sk_refcnt.
Result is that we might free a live TCP socket, as reported by
Dormando.
IPv4: Attempt to release TCP socket in state 1
Fix this issue by testing sk_state against TCP_LISTEN early, so that we
set TCP_MTU_REDUCED_DEFERRED on appropriate sockets (not a LISTEN one)
This bug was introduced in commit 563d34d05786

in commit 1355b704b9ba "bnx2x: consistent statistics after
internal driver reload" incorrectly fails to handle overflow
during addition of the lower 32-bit field of a stat.
This bug is present since 3.4-rc1 and should thus be considered
a candidate for stable 3.4+ releases.

KVM: VMX: Require KVM_SET_TSS_ADDR being called prior to running a VCPU
Very old user space (namely qemu-kvm before kvm-49) didn't set the TSS
base before running the VCPU. We always warned about this bug, but no
reports about users actually seeing this are known. Time to finally
remove the workaround that effectively prevented to call vmx_vcpu_reset
while already holding the KVM srcu lock.

The result of this conflict was that we again see CONFIG_VIRT_TO_BUS
on all ARM systems. This patch fixes up the problem and removes

arm64: fix padding computation in struct ucontext
The expression to compute the padding needed to fill the uc_sigmask field
to 1024 bits actually computes the padding needed for 1080 bits.
Fortunately, due to the 16-byte alignment of the following field
(uc_mcontext) the definition in glibc contains enough bytes of padding
after uc_sigmask so that the overall offsets and size match in both
definitions.

perf,x86: fix wrmsr_on_cpu() warning on suspend/resume
Commit 1d9d8639c063 ("perf,x86: fix kernel crash with PEBS/BTS after
suspend/resume") fixed a crash when doing PEBS performance profiling
after resuming, but in using init_debug_store_on_cpu() to restore the
DS_AREA mtrr it also resulted in a new WARN_ON() triggering.
init_debug_store_on_cpu() uses "wrmsr_on_cpu()", which in turn uses CPU
cross-calls to do the MSR update. Which is not really valid at the
early resume stage, and the warning is quite reasonable. Now, it all
happens to _work_, for the simple reason that smp_call_function_single()
ends up just doing the call directly on the CPU when the CPU number
matches, but we really should just do the wrmsr() directly instead.
This duplicates the wrmsr() logic, but hopefully we can just remove the
wrmsr_on_cpu() version eventually.
ubuf info allocator uses guest controlled head as an index,
so a malicious guest could put the same head entry in the ring twice,
and we will get two callbacks on the same value.
To fix use upend_idx which is guaranteed to be unique.

bnx2x: add missing napi deletion in error path
If the hardware initialization fails in bnx2x_nic_load() after adding
napi objects, they would not be deleted. A subsequent attempt to unload
the bnx2x module detects a corruption in the napi list.

net: ethernet: cpsw: fix usage of cpdma_check_free_tx_desc()
Commit fae50823d0 ("net: ethernet: davinci_cpdma: Add boundary for rx
and tx descriptors") introduced a function to check the current
allocation state of tx packets. The return value is taken into account
to stop the netqork queue on the adapter in case there are no free
slots.
However, cpdma_check_free_tx_desc() returns 'true' if there is room in
the bitmap, not 'false', so the usage of the function is wrong.

ethernet/tulip: DE4x5 needs VIRT_TO_BUS
The automated ARM build tests have shown that the tulip de4x5 driver
uses the old-style virt_to_bus() interface on some architectures.
Alpha, Sparc and PowerPC did not hit this problem, because they
use a different code path, and most other architectures actually
do provide VIRT_TO_BUS.

isdn: hisax: netjet requires VIRT_TO_BUS
Disabling CONFIG_VIRT_TO_BUS on ARM showed that the hisax netjet
driver depends on this deprecated functionality but is not
marked so in Kconfig.
Rather than adding ARM to the already long list of architectures
that this driver is broken on, this patch adds 'depends on
VIRT_TO_BUS' and removes the dependency on !SPARC, which is
also implied by that.

net: cdc_ncm, cdc_mbim: allow user to prefer NCM for backwards compatibility
commit bd329e1 ("net: cdc_ncm: do not bind to NCM compatible MBIM devices")
introduced a new policy, preferring MBIM for dual NCM/MBIM functions if
the cdc_mbim driver was enabled. This caused a regression for users
wanting to use NCM.
Devices implementing NCM backwards compatibility according to section
3.2 of the MBIM v1.0 specification allow either NCM or MBIM on a single
USB function, using different altsettings. The cdc_ncm and cdc_mbim
drivers will both probe such functions, and must agree on a common
policy for selecting either MBIM or NCM. Until now, this policy has
been set at build time based on CONFIG_USB_NET_CDC_MBIM.
Use a module parameter to set the system policy at runtime, allowing the
user to prefer NCM on systems with the cdc_mbim driver.

rtnetlink: Mask the rta_type when range checking

The commit is wrong as tiph points to the outer IPv4 header which is
installed at ipgre_header() and not the inner one which is protocol dependant.
This commit broke succesfully opennhrp which use PF_PACKET socket with
ETH_P_NHRP protocol. Additionally ssl_addr is set to the link-layer
IPv4 address. This address is written by ipgre_header() to the skb
earlier, and this is the IPv4 header tiph should point to - regardless
of the inner protocol payload.

This patch change the kernel VSID range so that we limit VSID_BITS to 37.
This enables us to support 64TB with 65 bit VA (37+28). Without this patch
we have boot hangs on platforms that only support 65 bit VA.
With this patch we now have proto vsid generated as below:
We first generate a 37-bit "proto-VSID". Proto-VSIDs are generated
from mmu context id and effective segment id of the address.
For user processes max context id is limited to ((1ul << 19) - 5)
for kernel space, we use the top 4 context ids to map address as below
0x7fffc - [ 0xc000000000000000 - 0xc0003fffffffffff ]
0x7fffd - [ 0xd000000000000000 - 0xd0003fffffffffff ]
0x7fffe - [ 0xe000000000000000 - 0xe0003fffffffffff ]

powerpc/ptrace: Fix brk.len used uninitialised
With some CONFIGS it's possible that in ppc_set_hwdebug, brk.len is
uninitialised before being used. It has been reported that GCC 4.2 will
produce the following error in this case:
arch/powerpc/kernel/ptrace.c:1479: warning: 'brk.len' is used uninitialized in this function
arch/powerpc/kernel/ptrace.c:1381: note: 'brk.len' was declared here
This patch corrects this.

Btrfs: fix warning of free_extent_map
Users report that an extent map's list is still linked when it's actually
going to be freed from cache.
The story is that
a) when we're going to drop an extent map and may split this large one into
smaller ems, and if this large one is flagged as EXTENT_FLAG_LOGGING which means
that it's on the list to be logged, then the smaller ems split from it will also
be flagged as EXTENT_FLAG_LOGGING, and this is _not_ expected.
b) we'll keep ems from unlinking the list and freeing when they are flagged with
EXTENT_FLAG_LOGGING, because the log code holds one reference.
The end result is the warning, but the truth is that we set the flag
EXTENT_FLAG_LOGGING only during fsync.
So clear flag EXTENT_FLAG_LOGGING for extent maps split from a large one.

- The GPIO descriptor work has exposed how broken the non-GPIOLIB bits

- The system.h split introduced a bug in llist.h for arches using
asm-generic/cmpxchg.h directly, which is currently only OpenRISC.
The patch here moves two defines from asm-generic/atomic.h to
asm-generic/cmpxchg.h to make things work as they should.
- The VIRT_TO_BUS selector was added for OpenRISC, but OpenRISC does
not have the virt_to_bus methods, so there's a patch to remove it
again.
