xen-netfront: delay gARP until backend switches to Connected

After a guest is live migrated, the xen-netfront driver emits a gratuitous
ARP message, so that networking hardware on the target host's subnet can
take notice, and public routing to the guest is re-established. However,
if the packet appears on the backend interface before the backend is added
to the target host's bridge, the packet is lost, and the migrated guest's
peers become unable to talk to the guest.

A sufficient two-parts condition to prevent the above is:

(1) ensure that the backend only moves to Connected xenbus state after its
hotplug scripts completed, ie. the netback interface got added to the
bridge; and

(2) ensure the frontend only queues the gARP when it sees the backend move
to Connected.

These two together provide complete ordering. Sub-condition (1) is already
satisfied by commit f942dc2552b8 in Linus' tree, based on commit
6b0b80ca7165 from [1].

In general, the full condition is sufficient, not necessary, because,
according to [2], live migration has been working for a long time without
satisfying sub-condition (2). However, after 6b0b80ca7165 was backported
to the RHEL-5 host to ensure (1), (2) still proved necessary in the RHEL-6
guest. This patch intends to provide (2) for upstream.

ipip, sit: copy parms.name after register_netdevice

Same fix as 731abb9cb2 for ipip and sit tunnel.
Commit 1c5cae815d removed an explicit call to dev_alloc_name in
ipip_tunnel_locate and ipip6_tunnel_locate, because register_netdevice
will now create a valid name, however the tunnel keeps a copy of the
name in the private parms structure. Fix this by copying the name back
after register_netdevice has successfully returned.

This shows up if you do a simple tunnel add, followed by a tunnel show:

$ sudo ip tunnel add mode ipip remote 10.2.20.211
$ ip tunnel
tunl0: ip/ip  remote any  local any  ttl inherit  nopmtudisc
tunl%d: ip/ip  remote 10.2.20.211  local any  ttl inherit

There is no obvious reason to add a default multicast route for loopback
devices, otherwise there would be a route entry whose dst.error set to
-ENETUNREACH that would blocking all multicast packets.

The problem is that the resulting routing table depends on the sequence
of interface's initialization and in some situation, that would block all
muticast packets. Suppose there are two interfaces on my computer
(lo and eth0), if we initailize 'lo' before 'eth0', the resuting routing
table(for multicast) would be

# ip -6 route show | grep ff00::
unreachable ff00::/8 dev lo metric 256 error -101
ff00::/8 dev eth0 metric 256

When sending multicasting packets, routing subsystem will return the first
route entry which with a error set to -101(ENETUNREACH).

I know the kernel will set the default ipv6 address for 'lo' when it is up
and won't set the default multicast route for it, but there is no reason to
stop 'init' program from setting address for 'lo', and that is exactly what
systemd did.

I am sure there is something wrong with kernel or systemd, currently I preferred
kernel caused this problem.

mtd: fsl_elbc_nand: set Nand flash page address to FBAR and FPAR correctly

If we use the Nand flash chip whose number of pages in a block is greater
than 64(for large page), we must treat the low bit of FBAR as being the
high bit of the page address due to the limitation of FCM, it simply uses
the low 6-bits (for large page) of the combined block/page address as the
FPAR component, rather than considering the actual block size.

PCI/PM/Runtime: make PCI traces quieter

When the runtime PM is activated on PCI, if a device switches state
frequently (e.g. an EHCI controller with autosuspending USB devices
connected) the PCI configuration traces might be very verbose in the
kernel log.  Let's guard those traces with DEBUG condition.

Convert from pci_scan_bus() to pci_scan_root_bus() and remove root bus
fixups.  This fixes the problem of "early" and "header" quirks
seeing incorrect root bus resources.

This arch was unusual because it filled in bus->resource[0..3] in
pcibios_init(), then overwrote them, applied io_space.offset and
checked for unset resources in pcibios_fixup_bus().  I moved all of
that to a new pci_controller_apertures() that we can use before
scanning the root bus.

x86/PCI: convert to pci_create_root_bus() and pci_scan_root_bus()

x86 has two kinds of PCI root bus scanning:

(1) ACPI-based, using _CRS resources.  This used pci_create_bus(), not
    pci_scan_bus(), because ACPI hotplug needed to split the
    pci_bus_add_devices() into a separate host bridge .start() method.

    This patch parses the _CRS resources earlier, so we can build a list of
    resources and pass it to pci_create_root_bus().

    Note that as before, we parse the _CRS even if we aren't going to use
    it so we can print it for debugging purposes.

(2) All other, which used either default resources (ioport_resource and
    iomem_resource) or information read from the hardware via amd_bus.c or
    similar.  This used pci_scan_bus().

    This patch converts x86_pci_root_bus_res_quirks() (previously called
    from pcibios_fixup_bus()) to x86_pci_root_bus_resources(), which builds
    a list of resources before we call pci_scan_root_bus().

    We also use x86_pci_root_bus_resources() if we have ACPI but are
    ignoring _CRS.

This doesn't change any functionality, but it makes a subsequent patch
slightly simpler.

pci_scan_bus(NULL, ...) and pci_scan_bus_parented() are identical except
that pci_scan_bus() also calls pci_bus_add_devices():

  pci_scan_bus_parented
    pci_create_bus
    pci_scan_child_bus

  pci_scan_bus
    pci_create_bus
    pci_scan_child_bus
    pci_bus_add_devices

All callers of pcibios_scan_root() call pci_bus_add_devices() explicitly,
and we don't pass a parent device, so we might as well use pci_scan_bus().

We currently read the CNB20LE aperture information in a PCI quirk,
which happens after we've already created the root bus.  This patch
changes it to read the apertures earlier so we can create the root
bus with the correct resources.

I believe the CNB20LE lives at "pci 0000:00:00" based on

This moves the truncate_pat_collision() call out of the pcibios_fixup_bus()
path so that when a future patch builds a list of root bus resources for
pci_create_bus(), it can use the truncated LMMIO range.

truncate_pat_collision() used to be called in this path:

    pci_scan_bus_parented
      pci_create_bus
      pci_scan_child_bus
        pcibios_fixup_bus
  lba_fixup_bus
    truncate_pat_collision

All of the PAT and lba_dev resource setup must be done before we call
pci_scan_bus_parented(), so it should be safe to move the
truncate_pat_collision() to just before pci_scan_bus_parented().
