mmc: tmio: check ILL_FUNC instead of CBSY

Some controllers need to check SD bus status when writing data.
Then, it checks ILL_FUNC bit on SD_INFO2 register,
and this method is controlled via TMIO_MMC_HAS_IDLE_WAIT flags.
Same method is required on tmio_mmc_data_irq() which will
be called after writing data.

Current driver is checking CBSY bit for this purpose,
but, some controllers doesn't have CBSY bit.
This patch checks ILL_FUNC bit instead of CBSY bit
if it has TMIO_MMC_HAS_IDLE_WAIT flags

An internal power state machine were beeing used to keep ->probe() and
->set_ios() in sync. Especially for handling specific scenarios while
using CONFIG_MMC_CLKGATE. Moreover dependency to CONFIG_MMC_CLKGATE
existed to handle runtime PM properly, which we moves away from here.

By removing the state machine and instead make ->set_ios() rely on the
information provided through the function's in-parameters, the code
becomes significantly simplier.

Additonally as a part of this rework we prepares for making the runtime
PM callbacks responsible of clock gating.

mmc: core: Use regulator_get_voltage() if OCR mask is empty.

The operation conditions register (OCR) stores the voltage
profile of the card, however the list of possible voltages
is restricted by the voltage range supported by the supply
used as VCC/VDD. So in mmc_vddrange_to_ocrmask() a OCR mask
is obtained to filter the not supported voltages, from the
value read in the host controller OCR register.

For fixed regulators, regulator_list_voltage() returns the
fixed output for the first selector but this doesn't happen
for switch (FET) regulators that obtain their voltage from
their parent supply. A call to regulator_get_voltage() is
needed in this case so the regulator core can return the
FET's parent supply voltage output.

This change is consistent with the fact that for other
fixed regulators (that are not FETs) the OCR mask is
returned even when mmc_regulator_set_ocr() checks if the
regulator is fixed before calling regulator_set_voltage().

Without this patch, the following warning is reported when
a FET is used as a vmmc-supply:

dwmmc_exynos 12220000.mmc: Failed getting OCR mask: -22

Some eMMC and SD cards implement a DSR register that allows to tune
raise/fall times and drive strength of the CMD and DATA outputs.
The values to use depend on the card in use and the host.
It might be needed to reduce the drive strength to prevent voltage peaks
above the host's specification.

Implement a 'dsr' devicetree property that allows to specify the value
to set the DSR to. For non-dt setups the new members of mmc_host can be
set by board code.

This patch was initially authored by Sascha Hauer. It contains
improvements authored by Markus Niebel and Uwe Kleine-König.

.set_uhs_signaling field is currently initialised twice once to the
arch specific callback pxav3_set_uhs_signaling, and also to the generic
sdhci_set_uhs_signaling callback.

This means that uhs is currently broken for this platform currently, as pxav3
has some special constriants which means it can't use the generic callback.

This happened in
commit 96d7b78cfc2f ("mmc: sdhci: convert sdhci_set_uhs_signaling() into a library function")
commit a702c8abb2a9 ("mmc: host: split up sdhci-pxa, create sdhci-pxav3.c")'

Fix this and hopefully prevent it happening in the future by ensuring named
initialisers always follow the declaration order in the structure definition.

when SDHCI_QUIRK_DATA_TIMEOUT_USES_SDCLK is set, timeout_clk is sdclk.
We need to update it when we change sdclk in sdhci_set_clock.
This allow to have a more precisse timeout and max_busy_timeout. This
can help for command that need a big busy wait (erase, ...).

When we wait for busy after sending a command, if there is
a timeout, we got SDHCI_INT_DATA_TIMEOUT flags.
Before this commit we got the message :
"Got data interrupt 0x00100000 even though no data  operation was in progress."
and we need to wait 10s that sdhci_timeout_timer expires.

mmc: dw_mmc: Make sure we don't get stuck when we get an error

If we happened to get a data error at just the wrong time the dw_mmc
driver could get into a state where it would never complete its
request.  That would leave the caller just hanging there.

We fix this two ways and both of the two fixes on their own appear to
fix the problems we've seen:

1. Fix a race in the tasklet where the interrupt setting the data
   error happens _just after_ we check for it, then we get a
   EVENT_XFER_COMPLETE.  We fix this by repeating a bit of code.
2. Fix it so that if we detect that we've got an error in the "data
   busy" state and we're not going to do anything else we end the
   request and unblock anyone waiting.

Make use of the MMC asynchronous request capability to prepare the
next DMA transfer request in parallel with the current transfer.
This is done by adding pre-request and post-request callbacks that are
used by the MMC framework during an active data transfer.

It should help reduce the impact of DMA preparation overhead on the SD
card performance.

Until now the MMC driver for JZ4740 SoC was relying on PIO mode only
for data transfers.
This patch allows the use of DMA for data trasnfers in addition to PIO
mode by relying on DMA Engine.

DMA tranfers performance might be further improved by taking advantage
of the asynchronous request capability of the MMC framework.

The code selecting a device for the sdhci host has been
continuously tweaked (4b711cb13843f5082e82970dd1e8031383134a65
"mmc: sdhci-pltfm: Add structure for host-specific data" and
a4d2177f00a5252d825236c5124bc1e9918bdb41 "mmc: sdhci-pltfm: dt
device does not pass parent to sdhci_alloc_host" while there
does not seem to be any reason to use platform device's parent
in the first place.

The comment saying "Some PCI-based MFD need the parent here"
seem to refer to Timberdale FPGA driver (the only MFD driver
registering SDHCI cell, drivers/mfd/timberdale.c) but again,
the only situation when parent device matter is runtime PM,
which is not implemented for Timberdale.

mmc: mmci: Add qcom dml support to the driver.

On Qualcomm APQ8064 SOCs, SD card controller has an additional glue
called DML (Data Mover Local/Lite) to assist dma transfers.
This hardware needs to be setup before any dma transfer is requested.
DML itself is not a DMA engine, its just a gule between the SD card
controller and dma controller.

Most of this code has been ported from qualcomm's 3.4 kernel.

This patch adds the code necessary to intialize the hardware and setup
before doing any dma transfers.

The example code provided with the i2c device interface documentation
won't compile since it uses the reserved word "register" to name a
variable.

The compiler fails with this error message:

 error: expected identifier or '(' before '=' token

                 ^

Rename the variable "register" to simply "reg" in the example code.

Another couple of typos has been fixed as well.

ufs: fix deadlocks introduced by sb mutex merge

Commit 0244756edc4b ("ufs: sb mutex merge + mutex_destroy") introduces
deadlocks in ufs_new_inode() and ufs_free_inode().
Most callers of that functions acqure the mutex by themselves and
ufs_{new,free}_inode() do that via lock_ufs(),
i.e we have an unavoidable double lock.

The patch proposes to resolve the issue by making sure that
ufs_{new,free}_inode() are not called with the mutex held.

Found by Linux Driver Verification project (linuxtesting.org).

 "The fixes all address recently discovered data corruption issues.

  The original Direct IO issue was discovered by Chris Mason @ Facebook
  on a production workload which mixed buffered reads with direct reads
  and writes IO to the same file.  The fix for that exposed other issues
  with page invalidation (exposed by millions of fsx operations) failing
  due to dirty buffers beyond EOF.

  Finally, the collapse_range code could also cause problems due to
  racing writeback changing the extent map while it was being shifted
  around.  The commits for that problem are simple mitigation fixes that
  prevent the problem from occuring.  A more robust fix for 3.18 that
  addresses the underlying problem is currently being worked on by
  Brian.

  Summary of fixes:
   - a direct IO read/buffered read data corruption
   - the associated fallout from the DIO data corruption fix
   - collapse range bugs that are potential data corruption issues"

timekeeping: Update timekeeper before updating vsyscall and pvclock

The update_walltime() code works on the shadow timekeeper to make the
seqcount protected region as short as possible. But that update to the
shadow timekeeper does not update all timekeeper fields because it's
sufficient to do that once before it becomes life. One of these fields
is tkr.base_mono. That stays stale in the shadow timekeeper unless an
operation happens which copies the real timekeeper to the shadow.

The update function is called after the update calls to vsyscall and
pvclock. While not correct, it did not cause any problems because none
of the invoked update functions used base_mono.

commit cbcf2dd3b3d4 (x86: kvm: Make kvm_get_time_and_clockread()
nanoseconds based) changed that in the kvm pvclock update function, so
the stale mono_base value got used and caused kvm-clock to malfunction.

Put the update where it belongs and fix the issue.

The function cleaning up an initialized event
was called from the "event_del" handler, instead
of being used as the "destroy" callback. In case of
events group allocation this caused NULL pointer
dereference (as events are added and deleted
multiple times then). Fixed now.

Dave Hansen reports a massive scalability regression in an uncontained
page fault benchmark with more than 30 concurrent threads, which he
bisected down to 05b843012335 ("mm: memcontrol: use root_mem_cgroup
res_counter") and pin-pointed on res_counter spinlock contention.

That change relied on the per-cpu charge caches to mostly swallow the
res_counter costs, but it's apparent that the caches don't scale yet.

Revert memcg back to bypassing res_counters on the root level in order
to restore performance for uncontained workloads.

Export sync_filesystem() for modular ->remount_fs() use

This patch changes sync_filesystem() to be EXPORT_SYMBOL().

The reason this is needed is that starting with 3.15 kernel, due to
Theodore Ts'o's commit 02b9984d6408 ("fs: push sync_filesystem() down to
the file system's remount_fs()"), all file systems that have dirty data
to be written out need to call sync_filesystem() from their
->remount_fs() method when remounting read-only.

As this is now a generically required function rather than an internal
only function it should be EXPORT_SYMBOL() so that all file systems can
call it.

The local nohz kick is currently used by perf which needs it to be
NMI-safe. Recent commit though (7d1311b93e58ed55f3a31cc8f94c4b8fe988a2b9)
changed its implementation to fire the local kick using the remote kick
API. It was convenient to make the code more generic but the remote kick
isn't NMI-safe.

ARM: dts: dra7-evm: Add vtt regulator support

DRA7 evm REV G and later boards uses a vtt regulator for DDR3
termination and this is controlled by gpio7_11. This gpio is
configured in boot loader. gpio7_11, which is only available only on
Pad A22, in previous boards, is connected only to an unused pad on
expansion connector EXP_P3 and is safe to be muxed as GPIO on all
DRA7-evm versions (without a need to spin off another dts file).

Since gpio7_11 is used to control VTT and should not be reset or kept
in idle state during boot up else VTT will be disconnected and DDR
gets corrupted. So, as part of this change, mark gpio7 as no-reset and
no-idle on init.

Both QSPI and GPMC-NAND share the same Pin (A8) from the SoC for Chip Select
functionality. So both can't be enabled simultaneously.

Disable QSPI node to prevent the pin conflict as well as
be similar to 3.12 release.

ARM: OMAP2+: gpmc: Don't complain if wait pin is used without r/w monitoring

For NAND read & write wait pin monitoring must be kept disabled as the
wait pin is only used to indicate NAND device ready status and not to
extend each read/write cycle.

So don't print a warning if wait pin is specified while read/write
monitoring is not in the device tree.

Sanity check wait pin number irrespective if read/write monitoring is
set or not.

am437x-gp-evm uses a NAND chip with page size 4096 bytes
and spare area of 225 bytes per page.

For such a setup it is preferrable to use BCH16 ECC scheme over
BCH8. This also makes it compatible with ROM code ECC scheme so
we can boot with NAND after flashing from kernel.

This reverts commit 150b8be3cda54412ad7b54f5392b513b25c0aaa7.

The I2C core's per-adapter locks can't protect from IRQs, so the driver still
needs a spinlock to protect the register accesses.
