ocfs2: avoid unaligned access to dqc_bitmap

The dqc_bitmap field of struct ocfs2_local_disk_chunk is 32-bit aligned,
but not 64-bit aligned.  The dqc_bitmap is accessed by ocfs2_set_bit(),
ocfs2_clear_bit(), ocfs2_test_bit(), or ocfs2_find_next_zero_bit().  These
are wrapper macros for ext2_*_bit() which need to take an unsigned long
aligned address (though some architectures are able to handle unaligned
address correctly)

So some 64bit architectures may not be able to access the dqc_bitmap
correctly.

This avoids such unaligned access by using another wrapper functions for
ext2_*_bit().  The code is taken from fs/ext4/mballoc.c which also need to
handle unaligned bitmap access.

If we handled an error condition, then nfs4_recovery_handle_error should
return '0' so that the state recovery thread can continue.
Also ensure that nfs4_check_lease() continues to abort if we haven't got
any credentials by having it return ENOKEY (which is not handled).

Whenever we free a slot, we know that the resulting xprt->num_reqs will

If we take the "try_again" goto, due to a checksum error,
the 'len' has already been truncated.  So we won't compute

bridge: master device stuck in no-carrier state forever when in user-stp mode

When in user-stp mode, bridge master do not follow state of its slaves, so
after the following sequence of events it can stuck forever in no-carrier
state:
1) turn stp off
2) put all slaves down - master device will follow their state and also go in
no-carrier state
3) turn stp on with bridge-stp script returning 0 (go to the user-stp mode)
Now bridge master won't follow slaves' state and will never reach running
state.

This patch solves the problem by making user-stp and kernel-stp behavior
similar regarding master following slaves' states.

This reverts commit df711fc9962b9491af2b92bd0d21ecbfefe4e5fa.

The commit added a reset-on-resume quirk because the NEC chipset stopped
responding to commands about 30 minutes after a system resume from
suspend.  We thought it was a chipset issue, but it turns out that the
xHCI driver was zeroing out the link TRB after a successful context
restore during resume.  The host controller would fall off the command
ring sometime later, causing it to not respond to new commands.

The link TRB issue has been fixed with commit
158886cd2cf4599e04f9b7e10cb767f5f39b14f1 "xHCI: fix bug in
xhci_clear_command_ring()", so revert the reset-on-resume quirk, as it's
not necessary.

Commit df711fc9962b9491af2b92bd0d21ecbfefe4e5fa was marked for stable
trees back to 2.6.37, but according to my mail, it has not made it into
Linus' tree or the stable trees yet.

xHCI: fix bug in xhci_clear_command_ring()

When system enters suspend, xHCI driver clears command ring by writing zero
to all the TRBs. However, this also writes zero to the Link TRB, and the ring
is mangled. This may cause driver accesses wrong memory address and the
result is unpredicted.

When clear the command ring, keep the last Link TRB intact, only clear its
cycle bit. This should fix the "command ring full" issue reported by Oliver
Neukum.

This should be backported to stable kernels as old as 2.6.37, since the
commit 89821320 "xhci: Fix command ring replay after resume" is merged.

Btrfs: fix meta data raid-repair merge problem

Commit 4a54c8c16 introduced raid-repair, killing the individual
readpage_io_failed_hook entries from inode.c and disk-io.c. Commit
4bb31e92 introduced new readahead code, adding a readpage_io_failed_hook to
disk-io.c.

The raid-repair commit had logic to disable raid-repair, if
readpage_io_failed_hook is set. Thus, the readahead commit effectively
disabled raid-repair for meta data.

This commit changes the logic to always attempt raid-repair when needed and
call the readpage_io_failed_hook in case raid-repair fails. This is much
more straight forward and should have been like that from the beginning.

Increase cpu topology change poll frequency if a change is anticipated.
Otherwise a user might be a bit confused to have to wait up to a minute
in order to see a change this should be visible immediatly.
However there is no guarantee that the change will happen during the
time frame the poll frequency is increased.

Another round of cleanup for entry[64].S, in particular the program check
handler looks more reasonable now. The code size for the 31 bit kernel
has been reduced by 616 byte and by 528 byte for the 64 bit version.
Even better the code is a bit faster as well.

There is no reason for the cpu-measurement-facility host id constant to
reside in the lowcore where space is precious. Use an entry in the literal
pool in HANDLE_SIE_INTERCEPT and a stack slot in sie64a.
While we are at it replace the id -1 with 0 to indicate host execution.

This patch disables the check for MACHINE_IS_VM when initializing the
pfault infrastructure. The code checks for successful completion of
diag 258 anyway, thus it's safe to try initialization on LPAR anyway.
This is needed to use pfault on kvm

[S390] topology: get rid of ifdefs

Remove all ifdefs from topology code and also only compile it for the
CONFIG_SCHED_BOOK case. The new code selects SCHED_MC if SCHED_BOOK is
selected. SCHED_MC without SCHED_BOOK is not possible anymore.
Furthermore various sysfs attributes are not available anymore for the
!SCHED_BOOK case. In particular all attributes that correspond to
CPU polarization.
But since all real world kernels have SCHED_BOOK selected anyway this
doesn't matter too much.

[S390] add support for physical memory > 4TB

The kernel address space of a 64 bit kernel currently uses a three level
page table and the vmemmap array has a fixed address and a fixed maximum
size. A three level page table is good enough for systems with less than
3.8TB of memory, for bigger systems four page table levels need to be
used. Each page table level costs a bit of performance, use 3 levels for
normal systems and 4 levels only for the really big systems.
To avoid bloating sparse.o too much set MAX_PHYSMEM_BITS to 46 for a
maximum of 64TB of memory.

If a driver wants to do command mode IO while CIO is doing
online path verification we ignore this request and provide
a fake irb when we are done and the driver can do IO again.

For transport mode IO we have no such mechanism, giving the
driver no other chance then to retry the action until we are
done. This is not very reliable.

Provide a fake irb for transport mode IO as well.

When a driver requests to do IO, we will adjust the mask of
paths to be used to exclude varied offline paths.

Drivers trying to do IO solely on paths which are online but some
way defective may lack the information to do proper error handling.

There is no reason to allow the usage of known to be broken paths.
Thus restrict the paths a ccw driver can use for IO to a subset of
the paths cio found usable (this also excludes offline paths).

git commit 20b40a794baf3b4b "signal race with restarting system calls"
added code to the poke_user/poke_user_compat to reset the system call
restart information in the thread-info if the PSW address is changed.
The purpose of that change has been to workaround old gdbs that do
not know about the REGSET_SYSTEM_CALL. It turned out that this is not
a good idea, it makes the behaviour of the debuggee dependent on the
order of specific ptrace call, e.g. the REGSET_SYSTEM_CALL register
set needs to be written last. And the workaround does not really fix
old gdbs, inferior calls on interrupted restarting system calls do not
work either way.

This patch makes sure we don't underindicate _PAGE_CHANGED in case
we have a race between an operation that changes the page and this
code path that hits us between page_get_storage_key and
page_set_storage_key. Note that we still have a potential
underindication on _PAGE_REFERENCED in the unlikely event that
the page was changed but not referenced _and_ someone references
the page in the race window. That's not considered to be a problem.

The verb command in stac92xx_post_suspend caused the audio to stop
working after resuming from S3 mode on HP laptops with the VREF-pin
mute-LED control.  Removing relevant post_suspend registering.

Although removing D3 on AFG is no optimal solution, the impact should
be small in comparison with the broken S3/S4.
