arm: kirkwood: convert db-88f6281/db-88f6282 to the Device Tree
This commit converts the Marvell DB-88F6281/DB-88F6282 board to the
Device Tree. In fact, the code was supporting two different boards:
one with the 6281 SoC variant, and one with the 6282 SoC variant. The
difference between the two being that the 6281 has one PCIe interface,
and the 6282 has two PCIe interfaces.
In order to handle that with the Device Tree, we create a
'kirkwood-db.dtsi' file that contains the definitions common to both
boards, and 'kirkwood-db-88f6281.dts' and 'kirkwood-db-88f6282.dts'
for the definitions specific to each board. This is similar to what is
done for the QNAP TS219 Kirkwood platform.
We have kept one single Kconfig option, just like it was before.

Now that the PCIe mvebu driver is usable on Kirkwood, use it instead
of the legacy PCIe code, since it allows to describe the PCIe
interfaces in the Device Tree.
Since it was the only device left that prevented this platform to use
the Device Tree only, we remove the board-nsa310.c file and the
related Kconfig/Makefile bits.

arm: kirkwood: move PCIe window init to legacy driver
Since we are going to enable the usage of the mvebu PCIe driver on
Kirkwood, we don't want the PCIe windows to be unconditionally created
by kirkwood_setup_wins(). Therefore, we move the PCIe window
initialization into the legacy PCIe driver
(arch/arm/mach-kirkwood/pcie.c).
The platforms using the legacy driver will see their windows
statically allocated by
arch/arm/mach-kirkwood/pcie.c:kirkwood_pcie_init(). The platforms
using the new driver in drivers/pci/ will see their windows
dynamically allocated directly by the driver.

pci: PCIe driver for Marvell Armada 370/XP systems
This driver implements the support for the PCIe interfaces on the
Marvell Armada 370/XP ARM SoCs. In the future, it might be extended to
cover earlier families of Marvell SoCs, such as Dove, Orion and
Kirkwood.
The driver implements the hw_pci operations needed by the core ARM PCI
code to setup PCI devices and get their corresponding IRQs, and the
pci_ops operations that are used by the PCI core to read/write the
configuration space of PCI devices.
Since the PCIe interfaces of Marvell SoCs are completely separate and
not linked together in a bus, this driver sets up an emulated PCI host
bridge, with one PCI-to-PCI bridge as child for each hardware PCIe
interface.
In addition, this driver enumerates the different PCIe slots, and for
those having a device plugged in, it sets up the necessary address
decoding windows, using the mvebu-mbus driver.

clk: mvebu: add more PCIe clocks for Armada XP
The current revision of the datasheet only mentions the gatable clocks
for the PCIe 0.0, 0.1, 0.2 and 0.3 interfaces, and forgot to mention
the ones for the PCIe 1.0, 1.1, 1.2, 1.3, 2.0 and 3.0
interfaces. After confirmation with Marvell engineers, this patch adds
the missing gatable clocks for those PCIe interfaces.
It also changes the name of the previously existing PCIe gatable
clocks, in order to match the naming using the datasheets.

The Armada 370 has two gatable clocks for each PCIe interface, and we
want both of them to be enabled. We therefore make one of the two
clocks a child of the other, as we did for the sataX and sataXlnk
clocks on Armada XP.

arm: mvebu: enable the third USB interface on OpenBlocks AX3
Besides the two "classic" USB interfaces with normal USB ports on the
front side, the PlatHome OpenBlocks AX3 uses the third USB interface
of the Marvell SoC in the mini-PCIe connector. This allows certain
mini-PCIe cards to expose parts of their functionality as a USB
peripheral.
This commit enables this third USB interface in the OpenBlocks AX3
Device Tree, and also adds comments on top of the two other USB
interfaces so that the Device Tree makes it clear which USB interface
at the SoC level matches which USB interface visible on the board.

ARM: dts: mvebu: Fix wrong the address reg value for the L2-cache node
During the conversion to the internal-regs' subnode, the L2-cache node
haven not been converted (due to a wrong choice made by myself during
the resolution of the merge conflict when I rebased the commit). This
leads to wrong address for L2 cache which prevent it to be used on
Armada 370. This commit fix the address reg of the e L2-cache node.

ARM: plat-orion: Fix num_resources and id for ge10 and ge11
When platform data were moved from arch/arm/mach-mv78xx0/common.c to
arch/arm/plat-orion/common.c with the commit "7e3819d ARM: orion:
Consolidate ethernet platform data", there were few typo made on
gigabit Ethernet interface ge10 and ge11. This commit writes back
their initial value, which allows to use this interfaces again.

This function can be used to parse the device and function number from a
standard 5-cell PCI resource. PCI_SLOT() and PCI_FUNC() can be used on

of/pci: Provide support for parsing PCI DT ranges property
This patch factors out common implementation patterns to reduce overall kernel
code and provide a means for host bridge drivers to directly obtain struct
resources from the DT's ranges property without relying on architecture specific
DT handling. This will make it easier to write archiecture independent host bridge
drivers and mitigate against further duplication of DT parsing code.
Additionally the implementation takes care of adjacent ranges and merges them
into a single range (as was the case with powerpc and microblaze).

arm: mvebu: fix the 'ranges' property to handle PCIe
Since 82a682676 ('ARM: dts: mvebu: Convert all the mvebu files to use
the range property') all the device nodes of Armada 370/XP are under a
common 'ranges' property that translates the device register addresses
into their absolute address, thanks to the base address of the
internal register space.
However, beyond just the register areas, there are also PCIe I/O and
memory regions, whose addresses should be properly translated. This
patch fixes the Armada 370 and XP ranges property to take PCIe into
account properly.

mach-kirkwood/board-dt.c defines its own OF matching table, but in
fact, it is the same to the default of_default_bus_match_table[] which
is already used by mach-mvebu, mach-dove and mach-orion5x.
This patch therefore makes mach-kirkwood use the same default OF
matching table.

Based on the current common functions for core clocks and clock
gating control, new common functions are joined in a single file.
Given the opportunity, names of functions and structs are unified,
and also a Kconfig entry is added.

Clock gates found on MVEBU SoCs get registered by a common function.
To allow specific SoCs to provide tweaks introduce flags to the clock
gate descriptor instead of filling up the common function SoC specific
tweaks.

Initially ARCH_REQUIRE_GPIOLIB was part of Thomas Petazzoni series
when he introduced the gpiolib support for mvebu: 93a59cf arm: mvebu: use GPIO support now that a driver is available
This commit was written to be applied for the ARCH_MVEBU which was
located in arch/arm/KConfig and was merged in 3.7.
In the same time Rob Herring moved the ARCH_MVEBU block to
arch/arm/mach-mvebu/Kconfig with this commit and also merged in 3.7: 387798b ARM: initial multiplatform support
Unfortunately the ARCH_REQUIRE_GPIOLIB have been lost during this
migration. This was not noticed until the v3.10-rc1, because mvebu as
part of ARCH_MULTIPLATFORM was always selected with ARCH_VEXPRESS, and
this architect selected ARCH_REQUIRE_GPIOLIB.
Since the following commit from Arnd: "883a106 ARM: default machine
descriptor for multiplatform", ARCH_VEXPRESS was then no more selected
by default with ARCH_MVEBU and it made appeared the lack of
ARCH_REQUIRE_GPIOLIB for mvebu. This commit added back the selection
of ARCH_REQUIRE_GPIOLIB for ARCH_MVEBU.

ARM: mvebu: Add support for NOR flash device on Armada XP-DB board
The Armada XP Development Board (DB-78460-BP) has a NOR flash device
connected to the Device Bus. This commit adds the device tree node
to support this device.
This SoC supports a flexible and dynamic decoding window allocation
scheme; but since this feature is still not implemented we need
to specify the window base address in the device tree node itself.
This base address has been selected in a completely arbitrary fashion.

'internal-reg' node, now the only region translated is the internal register's.
This makes the description of the hardware incomplete, for it lacks the
Device Bus childs address space.
In order to fix this, it's required to add a 'ranges' entry with a suitable
address space to map Device Bus childs, on a per-board basis.
This patch fixes the ranges property on the Armada XP GP board.

This patch adds the device tree node for si5351 clock generator
and the corresponding oscillator connected to it. It also limits
i2c frequency to 100kHz as there are bus locks reported on higher
frequencies.

[SCSI] iscsi class, qla4xxx: fix sess/conn refcounting when find fns are used
This fixes a bug where the iscsi class/driver did not do a put_device
when a sess/conn device was found. This also simplifies the interface
by not having to pass in some arguments that were duplicated and did
not need to be exported.

[SCSI] sas: unify the pointlessly separated enums sas_dev_type and sas_device_type
These enums have been separate since the dawn of SAS, mainly because the
latter is a procotol only enum and the former includes additional state
for libsas. The dichotomy causes endless confusion about which one you
should use where and leads to pointless warnings like this:
drivers/scsi/mvsas/mv_sas.c: In function 'mvs_update_phyinfo':
drivers/scsi/mvsas/mv_sas.c:1162:34: warning: comparison between 'enum sas_device_type' and 'enum sas_dev_type' [-Wenum-compare]
Fix by eliminating one of them. The one kept is effectively the sas.h
one, but call it sas_device_type and make sure the enums are all
properly namespaced with the SAS_ prefix.

[SCSI] pm80xx: Added SPCv/ve specific ids, variables and modify for SPC
Updated pci id table with device, vendor, subdevice and subvendor ids
for 8081, 8088, 8089 SAS/SATA controllers. Added SPCv/ve related macros.
Updated macros, hba info structure and other structures for SPCv/ve.
Update of structure and variable names for SPC hardware functionalities.

[SCSI] Handle MLQUEUE busy response in scsi_send_eh_cmnd
scsi_send_eh_cmnd() is calling queuecommand() directly, so
it needs to check the return value here.
The only valid return codes for queuecommand() are 'busy'
states, so we need to wait for a bit to allow the LLDD
to recover.

dm persistent data: add threshold callback to space map
Add a threshold callback function to the persistent data space map
interface for a subsequent patch to use.
dm-thin and dm-cache are interested in knowing when they're getting
low on metadata or data blocks. This patch introduces a new method
for registering a callback against a threshold.
