uprobes: Remove "verify" argument from set_orig_insn()
Nobody does set_orig_insn(verify => false), and I think nobody will.
Remove this argument. IIUC set_orig_insn(verify => false) was needed
to single-step without xol area.

Now that we have uprobe_dup_mmap() we can fold uprobe_reset_state()
into the new hook and remove it. mmput()->uprobe_clear_state() can't
be called before dup_mmap().

uprobes: Introduce MMF_HAS_UPROBES
Add the new MMF_HAS_UPROBES flag. It is set by install_breakpoint()
and it is copied by dup_mmap(), uprobe_pre_sstep_notifier() checks
it to avoid the slow path if the task was never probed. Perhaps it
makes sense to check it in valid_vma(is_register => false) as well.
This needs the new dup_mmap()->uprobe_dup_mmap() hook. We can't use
uprobe_reset_state() or put MMF_HAS_UPROBES into MMF_INIT_MASK, we
need oldmm->mmap_sem to avoid the race with uprobe_register() or
mmap() from another thread.
Currently we never clear this bit, it can be false-positive after
uprobe_unregister() or uprobe_munmap() or if dup_mmap() hits the
probed VM_DONTCOPY vma. But this is fine correctness-wise and has
no effect unless the task hits the non-uprobe breakpoint.

-EEXIST from install_breakpoint() no longer makes sense, all
callers should simply treat it as "success". Change the code
to return zero and simplify register_for_each_vma().

uprobes: Change uprobe_mmap() to ignore the errors but check fatal_signal_pending()
Once install_breakpoint() fails uprobe_mmap() "ignores" all other
uprobes and returns the error.
It was never really needed to to stop after the first error, and
in fact it was always wrong at least in -ENOTSUPP case.
Change uprobe_mmap() to ignore the errors and always return 0.
This is not what we want in the long term, but until we teach
the callers to handle the failure it would be better to remove
the pointless complications. And this doesn't look too bad, the
only "reasonable" error is ENOMEM but in this case the caller
should be oom-killed in the likely case or the system has more
serious problems.
However it makes sense to stop if fatal_signal_pending() == T.
In particular this helps if the task was oom-killed.
1. Kill dup_mmap()->uprobe_mmap(), it was only needed to calculate
new_mm->uprobes_state.count removed by the previous patch.
If the forking process has a pending uprobe (int3) in vma, it will
be copied by copy_page_range(), note that it checks vma->anon_vma
so "Don't copy ptes" is not possible after install_breakpoint()
which does anon_vma_prepare().
2. Remove is_swbp_at_addr() and "int count" in uprobe_mmap(). Again,
this was needed for uprobes_state.count.
As a side effect this fixes the bug pointed out by Srikar,
this code lacked the necessary put_uprobe().
3. uprobe_munmap() becomes a nop after the previous patch. Remove the
meaningless code but do not remove the helper, we will need it.

uprobes: Kill uprobes_state->count
uprobes_state->count is only needed to avoid the slow path in
uprobe_pre_sstep_notifier(). It is also checked in uprobe_munmap()
but ironically its only goal to decrement this counter. However,
it is very broken. Just some examples:
- uprobe_mmap() can race with uprobe_unregister() and wrongly
increment the counter if it hits the non-uprobe "int3". Note
that install_breakpoint() checks ->consumers first and returns
-EEXIST if it is NULL.
"atomic_sub() if error" in uprobe_mmap() looks obviously wrong
too.
- uprobe_munmap() can race with uprobe_register() and wrongly
decrement the counter by the same reason.
- Suppose an appication tries to increase the mmapped area via
sys_mremap(). vma_adjust() does uprobe_munmap(whole_vma) first,
this can nullify the counter temporarily and race with another
thread which can hit the bp, the application will be killed by
SIGTRAP.
- Suppose an application mmaps 2 consecutive areas in the same file
and one (or both) of these areas has uprobes. In the likely case
mmap_region()->vma_merge() suceeds. Like above, this leads to
uprobe_munmap/uprobe_mmap from vma_merge()->vma_adjust() but then
mmap_region() does another uprobe_mmap(resulting_vma) and doubles
the counter.
This patch only removes this counter and fixes the compile errors,
then we will try to cleanup the changed code and add something else
instead.

perf/x86: Fix microcode revision check for SNB-PEBS
The following patch makes the microcode update code path
actually invoke the perf_check_microcode() function and
thus potentially renabling SNB PEBS.
By default, CONFIG_MICROCODE_OLD_INTERFACE is
forced to Y in arch/x86/Kconfig. There is no
way to disable this. That means that the code
path used in arch/x86/kernel/microcode_core.c
did not include the call to perf_check_microcode().
Thus, even though the microcode was updated to a
version that fixes the SNB PEBS problem, perf_event
would still return EOPNOTSUPP when enabling precise
sampling.
This patch simply adds a call to perf_check_microcode()
in the call path used when OLD_INTERFACE=y.

LogFS maintains file system meta-data in special inodes. These
inodes are releated to each other, therefore they must be
destroyed in a proper order.
- "logfs: initialize the number of iovecs in bio"
LogFS used to panic when it was created on an encrypted LVM
volume. The patch fixes the problem by properly initializing
the BIO.
Plus a couple more:
- logfs: create a pagecache page if it is not present
- logfs: destroy the reserved inodes while unmounting

perf_event_interrupt() records an event only when an overflow occurs. But
this check for overflow is a simple 'if (val < 0)'.
Because the events are rolled back, this check for overflow fails and the
event is not recorded. perf_event_interrupt() later uses pmc_overflow() to
detect the overflow and resets the counters and the events are lost completely.
To properly detect the overflow of rolled back events, use pmc_overflow()
even when recording events.

powerpc: Fix personality handling in ppc64_personality()
Directly comparing current->personality against PER_LINUX32 doesn't work
in cases when any of the personality flags stored in the top three bytes
are used.
Directly forcefully setting personality to PER_LINUX32 or PER_LINUX
discards any flags stored in the top three bytes
Use personality() macro to compare only PER_MASK bytes and make sure that
we are setting only the bits that should be set, instead of overwriting
the whole value.

powerpc/dma-iommu: Fix IOMMU window check
Checking for device mask to cover the whole IOMMU table is too strict.
IOMMU allocators should handle mask constraint properly for each
allocation.
The patch enables to use old AirPort Extreme cards on PowerMacs with
more than 1GB of memory; without the patch the driver init fails with:

For powerpc BooKE and e200, singlestep is handled on the critical/dbg
exception stack. This causes current_thread_info() to fail for kgdb
internal, so previously We work around this issue by copying
the thread_info from the kernel stack before calling kgdb_handle_exception,
and copying it back afterwards.
But actually we don't do this properly. We should backup current_thread_info
then restore that when exit.

powerpc/kgdb: Do not set kgdb_single_step on ppc
The kgdb_single_step flag has the possibility to indefinitely
hang the system on an SMP system.
The x86 arch have the same problem, and that problem was fixed by
commit 8097551d9ab9b9e3630(kgdb,x86: do not set kgdb_single_step
on x86). This patch does the same behaviors as x86's patch.

If range.start or range.minlen is bigger than filesystem size, return
invalid value error. This fixes possible overflow in BTOBB macro when
passed value was nearly ULLONG_MAX.

kvm tools: Respect guest tcp window size
Respect guest tcp window size and stop sending tcp segments to guest
if guest's receive window is closed.
This fixes the TCP hang I'm seeing where guest and host are transferring
big chuck of data.
This problem was not triggered when guest and external host
communicates, probably because guest to external host communication
walks through real network and is much slower than guest and host
communication. Thus, guest's receive window has little chance to be
closed.

Using 'select' in Kconfig is hard, a platform cannot just
enable a driver without also making sure that its subsystem
is there. Also, there is no actual code dependency between
the platform and the gpio leds driver.
Without this patch, building without LEDS_CLASS esults in:

ARM: imx: build i.MX6 functions only when needed
The head-v7.S contains a call to the generic cpu_suspend function,
which is only available when selected by the i.MX6 code. As
pointed out by Shawn Guo, i.MX5 does not actually use any
functions defined in head-v7.S. It is also needed only for
the i.MX6 power management code and for the SMP code, so
we can restrict building this file to situations in which
at least one of those two is present.
Finally, other platforms with a similar file call it headsmp.S,
so we can rename it to the same for consistency.
Without this patch, building imx5 standalone results in:

This adds the ftrace hook at the beginning of the function before a
frame is set up, and allows the function callbacks to be able to access
parameters. As kprobes now can use function tracing (at least on x86)
this speeds up the kprobe hooks that are at the beginning of the
function.

ftrace: Do not test frame pointers if -mfentry is used
The function graph has a test to check if the frame pointer is
corrupted, which can happen with various options of gcc with mcount.
But this is not an issue with -mfentry as -mfentry does not need nor use
frame pointers for function graph tracing.

ftrace: Add -mfentry to Makefile on function tracer
Thanks to Andi Kleen, gcc 4.6.0 now supports -mfentry for x86
(and hopefully soon for other archs). What this does is to have
the function profiler start at the beginning of the function
instead of after the stack is set up. As plain -pg (mcount) is
called after the stack is set up, and in some cases can have issues
with the function graph tracer. It also requires frame pointers to
be enabled.
The -mfentry now calls __fentry__ at the beginning of the function.
This allows for compiling without frame pointers and even has the
ability to access parameters if needed.
If the architecture and the compiler both support -mfentry then
use that instead.

ftrace: Make recordmcount.c handle __fentry__
With gcc 4.6.0 the -mfentry feature places the function profiling
call at the start of the function. When this is used, the call is
to __fentry__ and not mcount.
Change recordmcount.c to record both callers to __fentry__ and
mcount.

ARM: imx: fix ksz9021rn_phy_fixup
The ksz9021rn_phy_fixup and mx6q_sabrelite functions try to
set up an ethernet phy if they can. They do check whether
phylib is enabled, but unfortunately the functions can only
be called from platform code if phylib is builtin, not
if it is a module

ARM: omap: allow building omap44xx without SMP
The new omap4 cpuidle implementation currently requires
ARCH_NEEDS_CPU_IDLE_COUPLED, which only works on SMP.
This patch makes it possible to build a non-SMP kernel
for that platform. This is not normally desired for
end-users but can be useful for testing.
Without this patch, building rand-0y2jSKT results in:

tools lib traceevent: Fix off-by-one bug in pevent_strerror()
As pevent_errno is defined using PEVENT_ERRORS which uses _PE macro
magic, the first errno is bigger than __PEVENT_ERRNO_START by 1. So we

xen/setup: Fix one-off error when adding for-balloon PFNs to the P2M.
When we are finished with return PFNs to the hypervisor, then
populate it back, and also mark the E820 MMIO and E820 gaps
as IDENTITY_FRAMEs, we then call P2M to set areas that can
be used for ballooning. We were off by one, and ended up
over-writting a P2M entry that most likely was an IDENTITY_FRAME.
For example:
1-1 mapping on 40000->40200
1-1 mapping on bc558->bc5ac
1-1 mapping on bc5b4->bc8c5
1-1 mapping on bc8c6->bcb7c
1-1 mapping on bcd00->100000
Released 614 pages of unused memory
Set 277889 page(s) to 1-1 mapping
Populating 40200-40466 pfn range: 614 pages added
=> here we set from 40466 up to bc559 P2M tree to be
INVALID_P2M_ENTRY. We should have done it up to bc558.
The end result is that if anybody is trying to construct
a PTE for PFN bc558 they end up with ~PAGE_PRESENT.

MIPS: pci-ar724x: avoid data bus error due to a missing PCIe module
If the controller has no PCIe module attached, accessing of the device
configuration space causes a data bus error. Avoid this by checking the
status of the PCIe link in advance, and indicate an error if the link
is down.
The binding doc and dts use properties "fsl,{cd,wp}-internal" while
esdhc driver uses "fsl,{cd,wp}-controller". Fix binding doc and dts
to get them match driver code.

ARM: imx6: spin the cpu until hardware takes it down
Though commit 602bf40 (ARM: imx6: exit coherency when shutting down
a cpu) improves the stability of imx6q cpu hotplug a lot, there are
still hangs seen with a more stressful hotplug testing.
It's expected that once imx_enable_cpu(cpu, false) is called, the cpu
will be taken down by hardware immediately, and the code after that
will not get any chance to execute. However, this is not always the
case from the testing. The cpu could possibly be alive for a few
cycles before hardware actually takes it down. So rather than letting
cpu execute some code that could cause a hang in these cycles, let's
make the cpu spin there and wait for hardware to take it down.

block: replace __getblk_slow misfix by grow_dev_page fix
Commit 91f68c89d8f3 ("block: fix infinite loop in __getblk_slow")
is not good: a successful call to grow_buffers() cannot guarantee
that the page won't be reclaimed before the immediate next call to
__find_get_block(), which is why there was always a loop there.
Yesterday I got "EXT4-fs error (device loop0): __ext4_get_inode_loc:3595:
inode #19278: block 664: comm cc1: unable to read itable block" on console,
which pointed to this commit.
I've been trying to bisect for weeks, why kbuild-on-ext4-on-loop-on-tmpfs
sometimes fails from a missing header file, under memory pressure on
ppc G5. I've never seen this on x86, and I've never seen it on 3.5-rc7
itself, despite that commit being in there: bisection pointed to an
irrelevant pinctrl merge, but hard to tell when failure takes between
18 minutes and 38 hours (but so far it's happened quicker on 3.6-rc2).
(I've since found such __ext4_get_inode_loc errors in /var/log/messages
from previous weeks: why the message never appeared on console until
yesterday morning is a mystery for another day.)
Revert 91f68c89d8f3, restoring __getblk_slow() to how it was (plus
a checkpatch nitfix). Simplify the interface between grow_buffers()
and grow_dev_page(), and avoid the infinite loop beyond end of device
by instead checking init_page_buffers()'s end_block there (I presume
that's more efficient than a repeated call to blkdev_max_block()),
returning -ENXIO to __getblk_slow() in that case.
And remove akpm's ten-year-old "__getblk() cannot fail ... weird"
comment, but that is worrying: are all users of __getblk() really
now prepared for a NULL bh beyond end of device, or will some oops??

x86/smp: Don't ever patch back to UP if we unplug cpus
We still patch SMP instructions to UP variants if we boot with a
single CPU, but not at any other time. In particular, not if we
unplug CPUs to return to a single cpu.
Paul McKenney points out:
mean offline overhead is 6251/48=130.2 milliseconds.
If I remove the alternatives_smp_switch() from the offline
path [...] the mean offline overhead is 550/42=13.1 milliseconds
Basically, we're never going to get those 120ms back, and the
code is pretty messy.
We get rid of:
1) The "smp-alt-once" boot option. It's actually "smp-alt-boot", the
documentation is wrong. It's now the default.
2) The skip_smp_alternatives flag used by suspend.
3) arch_disable_nonboot_cpus_begin() and arch_disable_nonboot_cpus_end()
which were only used to set this one flag.

hwmon: (asus_atk0110) Add quirk for Asus M5A78L
The old interface is bugged and reads the wrong sensor when retrieving
the reading for the chassis fan (it reads the CPU sensor); the new
interface works fine.
Limit the access to userspace only on the BSP where we load the
container, verify the patches in it and put them in the patch cache.
Then, at application time, we lookup the correct patch in the cache and
use it.
When we need to reload the userspace container, we do that over the
reload interface:
echo 1 > /sys/devices/system/cpu/microcode/reload
which reloads (a possibly newer) container from userspace and applies
then the newest patches from there.

x86, microcode, AMD: Add a small, per-family patches cache
This is a trivial cache which collects all ucode patches for the current
family of CPUs on the system. If a newer patch appears due to the
container file being updated in userspace, we replace our cached version
with the new one.
We search the equivalence table using the CPUID(1) signature of the
CPU in order to get the equivalence ID of the patch which we need to
apply. Add a function which does the reverse - it will be needed in
later patches.
While at it, pull the other equiv table function up in the file so that
it can be used by other functionality without forward declarations.
x86, microcode: Add a refresh firmware flag to ->request_microcode_fw
This is done in preparation for teaching the ucode driver to either load
a new ucode patches container from userspace or use an already cached
version. No functionality change in this patch.
x86, microcode: Cleanup cpu hotplug notifier callback
Mask out CPU_TASKS_FROZEN bit so that all _FROZEN cases can be dropped.
Also, add some more comments as to why CPU_ONLINE falls through to
CPU_DOWN_FAILED (no break), and for the CPU_DEAD case. Realign debug
printks better.
