firmware loader: allow builtin firmware load even if usermodehelper is disabled

In commit a144c6a6c924 ("PM: Print a warning if firmware is requested
when tasks are frozen") we not only printed a warning if somebody tried
to load the firmware when tasks are frozen - we also failed the load.

But that check was done before the check for built-in firmware, and then
when we disallowed usermode helpers during bootup (commit 288d5abec831:
"Boot up with usermodehelper disabled"), that actually means that
built-in modules can no longer load their firmware even if the firmware
is built in too.  Which used to work, and some people depended on it for
the R100 driver.

So move the test for usermodehelper_is_disabled() down, to after
checking the built-in firmware.

This should fix:

The NFSv4 spec does not specify that the server must repeat that error,
so in order to avoid having the delegations revoked, we should handle
it immediately.

Also note that NFS4ERR_CB_PATH_DOWN does in fact renew the lease...

NFSv4: The NFSv4.0 client must send RENEW calls if it holds a delegation

RFC3530 states that if the client holds a delegation, then it is obliged
to continue to send RENEW calls once every lease period in order to allow
the server to return NFS4ERR_CB_PATH_DOWN if the callback path is
unreachable.

This is not required for NFSv4.1, since the server can at any time set
the SEQ4_STATUS_CB_PATH_DOWN_SESSION in any SEQUENCE operation.

it configures parameters like sifs, slottime, acktimeout in

carl9170: Fix mismatch in carl9170_op_set_key mutex lock-unlock

If is_main_vif(ar, vif) reports that we have to fall back
to software encryption, we goto err_softw; before locking ar->mutex.
As a result, we have unprotected call to carl9170_set_operating_mode
and unmatched mutex_unlock.

The patch fix the issue by adding mutex_lock before goto.

Found by Linux Driver Verification project (linuxtesting.org).

PSW restart can be triggered on offline CPUs. If this happens, currently
the PSW restart code fails, because functions like smp_processor_id()
do not work on offline CPUs. This patch fixes this as follows:

If PSW restart is triggered on an offline CPU, the PSW restart (sigp restart)
is done a second time on another CPU that is online and the old CPU is
stopped afterwards.

[S390] kdump: Add size to elfcorehdr kernel parameter

Currently only the address of the pre-allocated ELF header is passed with
the elfcorehdr= kernel parameter. In order to reserve memory for the header
in the 2nd kernel also the size is required. Current kdump architecture
backends use different methods to do that, e.g. x86 uses the memmap= kernel
parameter. On s390 there is no easy way to transfer this information.
Therefore the elfcorehdr kernel parameter is extended to also pass the size.
This now can also be used as standard mechanism by all future kdump
architecture backends.

The syntax of the kernel parameter is extended as follows:

elfcorehdr=[size[KMG]@]offset[KMG]

This change is backward compatible because elfcorehdr=size is still allowed.

On s390 there is a different KEXEC_CONTROL_MEMORY_LIMIT for the normal and
the kdump kexec case. Therefore this patch introduces a new macro
KEXEC_CRASH_CONTROL_MEMORY_LIMIT. This is set to
KEXEC_CONTROL_MEMORY_LIMIT for all architectures that do not define
KEXEC_CRASH_CONTROL_MEMORY_LIMIT.

The main purpose for PSW restart will be kdump. Therefore customers will
issue "system restart" for creating a dump. If kdump is not enabled,
currently "PSW restart" will reboot the system and then no dump can
be created any more. In order to still allow a manual stand-alone dump in
the case a user issues "PSW restart" on a system that has not enabled
kdump we now stop the system.

When IPL'ing from a block device and an NSS should be created we must
make sure that the kernel image and the initrd are in different 1MB
segments. Otherwise creating the NSS will fail.
So we make sure the initrd is 4MB behind the end of the kernel image
like we do already when IPL via the VM reader is performed.

Add runtime PM support to the core Primecell driver, following the PCI
model of how this is done.

Rather than having every driver fiddle about with enabling runtime PM,
that's dealt with in the core and instead, drivers just do a put() in
their probe and a balancing get() in their remove function to activate
runtime PM for the device.

As we're dealing with enabling runtime PM in the core, fix up spi-pl022
as it must not enable and disable runtime PM itself anymore.

1. Remove device database entry (ddb) state.
2. Remove device database (DDB) list building.
                With open-iscsi integration the logins to the target devices are
        handled by the user space. So the information of target is now
        maintained in the iscsi_session object. This is handled at
        libiscsi level so there is no need to maintain a list of DDBs in
        the qla4xxx LLD.
3. qla4xxx: Remove add_device_dynamically.
                Since autologin in FW is disabled with open-iscsi integration,
        driver will never get an AEN for which driver has not requested
        a DDB index. So remove the add_device_dynamically function.
4. Remove qla4xxx_tgt_dscvr
        Since firmware autologin is disabled this function will not work.
        Now user has the ability to do the target discovery and login to
        each target individually. Firwmare will not do the login on its own.
5. Remove relogin related code
        All relogin is handled by userspace now. qla4xxx just need to
        notify userspace of a connection failure, this triggers the
        relogin.
6. Remove add_session and alloc_session
        Now qla4xxx uses iscsi_session_setup that would do the necessary
        allocations for session and ddb_entry.

Add scsi_transport_iscsi hooks in qla4xxx to support
iSCSI session management using iscsiadm.

This patch is based on discussion here

Now users can use iscsiadm to do target discovery and do login/logout to
individual targets using the qla4xxx iSCSI class interface.

This patch leaves some dead code, but to make it easier to review
we are leaving and in the next patch we will remove that old code.

V2 - NOTE: Added code to avoid waiting for AEN during login/logout
in the driver, instead added a kernel to user event
to notify iscsid about login status. Because of this
iscsid will not get blocked.

Offload drivers like qla4xxx will offload the sending of the login/logout
pdus still, so this patch adds iscsi_conn_login_event which is
used by these types of drivers to notify userspace that the connection
has changed state.

It also adds a iscsi_is_session_online helper so the lld
can query the sessions state field.

The iscsi class currently does not support writable sysfs
attrs for LLD sysfs settings. This patch converts the
iscsi class and driver's host attrs to use the attribute
container sysfs group and the sysfs group's is_visible callout
to be able to support readable or writable sysfs attrs.
