Use the recently-added bio front_pad field to allocate struct dm_target_io.
Prior to this patch, dm_target_io was allocated from a mempool. For each
dm_target_io, there is exactly one bio allocated from a bioset.
This patch merges these two allocations into one allocation: we create a
bioset with front_pad equal to the size of dm_target_io so that every
bio allocated from the bioset has sizeof(struct dm_target_io) bytes
before it. We allocate a bio and use the bytes before the bio as
dm_target_io.
466e69b8b03b8c1987367912782bc12988ad8794 dropped busmaster enable from the
global drm code and moved it to the individual drivers, but missed the savage
driver. So, this re-adds busmaster enable to the savage driver, fixing the
regression.

This merge also removes a new use of __cancel_delayed_work(), and

functions" fixup, though.

workqueue: avoid using deprecated functions
The network merge brought in a few users of functions that got
deprecated by the workqueue cleanups: the 'system_nrt_wq' is now the
same as the regular system_wq, since all workqueues are now non-
reentrant.
Similarly, remove one use of flush_work_sync() - the regular
flush_work() has become synchronous, and the "_sync()" version is thus
deprecated as being superfluous.
Don't check MAY_WRITE as a newly created file may not have write mode bits,
but POSIX allows the creating process to write regardless.
This is ok because NFSv4 OPEN ops handle write permissions correctly -
the ACCESS in the OPEN compound is to differentiate READ v EXEC permissions.
Fixes a regression due to commit 6168f62c (NFSv4: Add ACCESS operation to
OPEN compound)
iscsit: remove incorrect unlock in iscsit_build_sendtargets_resp
Fix a potential multiple spin-unlock -> deadlock scenario during the
overflow check within iscsit_build_sendtargets_resp() as found by
sparse static checking.
