FIX: md: process hangs at wait_barrier after 0->10 takeover
Following symptoms were observed:
1. After raid0->raid10 takeover operation we have array with 2
missing disks.
When we add disk for rebuild, recovery process starts as expected
but it does not finish- it stops at about 90%, md126_resync process
hangs in "D" state.
2. Similar behavior is when we have mounted raid0 array and we
execute takeover to raid10. After this when we try to unmount array-
it causes process umount hangs in "D"
In scenarios above processes hang at the same function- wait_barrier
in raid10.c.
Process waits in macro "wait_event_lock_irq" until the
"!conf->barrier" condition will be true.
In scenarios above it never happens.
Reason was that at the end of level_store, after calling pers->run,
we call mddev_resume. This calls pers->quiesce(mddev, 0) with
RAID10, that calls lower_barrier.
However raise_barrier hadn't been called on that 'conf' yet,
so conf->barrier becomes negative, which is bad.
This patch introduces setting conf->barrier=1 after takeover
operation. It prevents to become barrier negative after call
lower_barrier().

batman-adv: Linearize fragment packets before merge
We access the data inside the skbs of two fragments directly using memmove
during the merge. The data of the skb could span over multiple skb pages. An
direct access without knowledge about the pages would lead to an invalid memory
access.

md_make_request: don't touch the bio after calling make_request
md_make_request was calling bio_sectors() for part_stat_add
after it was calling the make_request function. This is
bad because the make_request function can free the bio and
because the bi_size field can change around.
The fix here was suggested by Jens Axboe. It saves the
sector count before the make_request call. I hit this
with CONFIG_DEBUG_PAGEALLOC turned on while trying to break
his pretty fusionio card.

CRED: Fix memory and refcount leaks upon security_prepare_creds() failure
In prepare_kernel_cred() since 2.6.29, put_cred(new) is called without
assigning new->usage when security_prepare_creds() returned an error. As a
result, memory for new and refcount for new->{user,group_info,tgcred} are
leaked because put_cred(new) won't call __put_cred() unless old->usage == 1.
Fix these leaks by assigning new->usage (and new->subscribers which was added
in 2.6.32) before calling security_prepare_creds().

CRED: Fix BUG() upon security_cred_alloc_blank() failure
In cred_alloc_blank() since 2.6.32, abort_creds(new) is called with
new->security == NULL and new->magic == 0 when security_cred_alloc_blank()
returns an error. As a result, BUG() will be triggered if SELinux is enabled
or CONFIG_DEBUG_CREDENTIALS=y.
If CONFIG_DEBUG_CREDENTIALS=y, BUG() is called from __invalid_creds() because
cred->magic == 0. Failing that, BUG() is called from selinux_cred_free()
because selinux_cred_free() is not expecting cred->security == NULL. This does
not affect smack_cred_free(), tomoyo_cred_free() or apparmor_cred_free().
Fix these bugs by
(1) Set new->magic before calling security_cred_alloc_blank().
(2) Handle null cred->security in creds_are_invalid() and selinux_cred_free().

x25: possible skb leak on bad facilities
Originally x25_parse_facilities returned
-1 for an error
0 meaning 0 length facilities
>0 the length of the facilities parsed.
5ef41308f94dc ("x25: Prevent crashing when parsing bad X.25 facilities") introduced more
error checking in x25_parse_facilities however used 0 to indicate bad parsing a6331d6f9a429 ("memory corruption in X.25 facilities parsing") followed this further for
DTE facilities, again using 0 for bad parsing.
The meaning of 0 got confused in the callers.
If the facilities are messed up we can't determine where the data starts.
So patch makes all parsing errors return -1 and ensures callers close and don't use the skb further.

mac80211: fix the skb cloned check in the tx path
Using skb_header_cloned to check if it's safe to write to the skb is not
enough - mac80211 also touches the tailroom of the skb.
Initially this check was only used to increase a counter, however this
commit changed the code to also skip skb data reallocation if no extra
head/tailroom was needed:
Re-enable the interrupt when it occurs to see all transitions.

microblaze: Fix msr instruction detection
Fix msr instructions detection. The current code
just use msrclr for loading msr content and compare
it with proper MSR content. If msrclr is not implemented
r8 contains pc address.
Previous code wanted to use MSR carry bit but if msrclr

ext4: Fix data corruption with multi-block writepages support
This fixes a corruption problem with the multi-block
writepages submittal change for ext4, from commit bd2d0210cf22f2bd0cef72eb97cf94fc7d31d8cc ("ext4: use bio
layer instead of buffer layer in mpage_da_submit_io").
(Note that this corruption is not present in 2.6.37 on
ext4, because the corruption was detected after the
feature was merged in 2.6.37-rc1, and so it was turned
off by adding a non-default mount option,
mblk_io_submit. With this commit, which hopefully
fixes the last of the bugs with this feature, we'll be
able to turn on this performance feature by default in
2.6.38, and remove the mblk_io_submit option.)
The ext4 code path to bundle multiple pages for
writeback in ext4_bio_write_page() had a bug: we should
be clearing buffer head dirty flags *before* we submit
the bio, not in the completion routine.
The patch below was tested on 2.6.37 under KVM with the
postgresql script which was submitted by Jon Nelson as
documented in commit 1449032be1.
Without the patch, I'd hit the corruption problem about
50-70% of the time. With the patch, I executed the
script > 100 times with no corruption seen.
I also fixed a bug to make sure ext4_end_bio() doesn't
dereference the bio after the bio_put() call.

ALSA: AACI: allow writes to MAINCR to take effect
The AACI TRM requires the MAINCR enable bit to be held zero for two
bitclk cycles plus three apb_pclk cycles. Use a delay of 1us to
ensure this.
Ensure that writes to MAINCR to change the addressed codec only happen
when required, and that they take effect in a similar manner to the
above, otherwise we seem to occasionally have stuck slot busy bits.

x86, nx: Mark the ACPI resume trampoline code as +x
We reserve lowmem for the things that need it, like the ACPI
wakeup code, way early to guarantee availability. This happens
before we set up the proper pagetables, so set_memory_x() has no
effect.
Until we have a better solution, use an initcall to mark the
wakeup code executable.

powerpc: Fix hcall tracepoint recursion
Spinlocks on shared processor partitions use H_YIELD to notify the
hypervisor we are waiting on another virtual CPU. Unfortunately this means
the hcall tracepoints can recurse.
The patch below adds a percpu depth and checks it on both the entry and
exit hcall tracepoints.

powerpc/numa: Add length when creating OF properties via VPHN
The rest of the NUMA code expects an OF associativity property with
the first cell containing the length. Without this fix all topology changes
cause us to misparse the property and put the cpu into node 0.

The hypervisor uses unsigned 1 byte counters to signal topology changes to
the OS. Since they can wrap we need to check for any difference, not just if
the hypervisor count is greater than the previous count.

powerpc/numa: Only use active VPHN count fields
VPHN supports up to 8 distance fields but the number of entries in
ibm,associativity-reference-points signifies how many are in use.
Don't look at all the VPHN counts, only distance_ref_points_depth
worth.
Since we already cap our distance metrics at MAX_DISTANCE_REF_POINTS,
use that to size the VPHN arrays and add a BUILD_BUG_ON to avoid it growing
larger than the VPHN maximum of 8.

Some of those functions try to adjust the CPU features, for example
to remove NAP support on some revisions. However, they seem to use
r5 as an index into the CPU table entry, which might have been right
a long time ago but no longer is. r4 is the right register to use.
This probably caused some off behaviours on some PowerMac variants
using 750cx or 7455 processor revisions.

powerpc: Pass the right cpu_spec to ->setup_cpu() on 64-bit
When calling setup_cpu() on 64-bit, we pass a pointer to the
cputable entry we have found. This used to be fine when cur_cpu_spec
was a pointer to that entry, but nowadays, we copy the entry into
a separate variable, and we do so before we call the setup_cpu()
callback. That means that any attempt by that callback at patching
the CPU table entry (to adjust CPU features for example) will patch
the wrong table.

Prevent packets duplication for frames targeting FCoE L2 ring:
packets were arriving to stack from both L2 RSS and from FCoE
L2 in a promiscuous mode.
Configure FCoE L2 ring to DROP_ALL rx mode, when interface is
configured to PROMISC, and to accept only unicast frames, when
interface is configured to ALL_MULTI.

Btrfs: exclude super blocks when we read in block groups
This has been resulting in a BUT_ON(ret) after btrfs_reserve_extent in
btrfs_cow_file_range. The reason is we don't actually calculate the bytes_super
for a block group until we go to cache it, which means that the space_info can
hand out reservations for space that it doesn't actually have, and we can run
out of data space. This is also a problem if you are using space caching since
we don't ever calculate bytes_super for the block groups. So instead everytime
we read a block group call exclude_super_stripes, which calculates the
bytes_super for the block group so it can be left out of the space_info. Then
whenever caching completes we just call free_excluded_extents so that the super
excluded extents are freed up. Also if we are unmounting and we hit any block
groups that haven't been cached we still need to call free_excluded_extents to
make sure things are cleaned up properly. Thanks,

Btrfs: make sure search_bitmap finds something in remove_from_bitmap
When we're cleaning up the tree log we need to be able to remove free space from
the block group. The problem is if that free space spans bitmaps we would not
find the space since we're looking for too many bytes. So make sure the amount
of bytes we search for is limited to either the number of bytes we want, or the
number of bytes left in the bitmap. This was tested by a user who was hitting
the BUG() after search_bitmap. With this patch he can now mount his fs.

x86-32: Make sure the stack is set up before we use it
Since checkin ebba638ae723d8a8fc2f7abce5ec18b688b791d7 we call
verify_cpu even in 32-bit mode. Unfortunately, calling a function
means using the stack, and the stack pointer was not initialized in
the 32-bit setup code! This code initializes the stack pointer, and
simplifies the interface slightly since it is easier to rely on just a
pointer value rather than a descriptor; we need to have different
values for the segment register anyway.
This retains start_stack as a virtual address, even though a physical
address would be more convenient for 32 bits; the 64-bit code wants
the other way around...

cifs: don't send an echo request unless NegProt has been done
When the socket to the server is disconnected, the client more or less
immediately calls cifs_reconnect to reconnect the socket. The NegProt
and SessSetup however are not done until an actual call needs to be
made.
With the addition of the SMB echo code, it's possible that the server
will initiate a disconnect on an idle socket. The client will then
reconnect the socket but no NegotiateProtocol request is done. The
SMBEcho workqueue job will then eventually pop, and an SMBEcho will be
sent on the socket. The server will then reject it since no NegProt was
done.
The ideal fix would be to either have the socket not be reconnected
until we plan to use it, or to immediately do a NegProt when the
reconnect occurs. The code is not structured for this however. For now
we must just settle for not sending any echoes until the NegProt is
done.

bridge: Don't put partly initialized fdb into hash
The fdb_create() puts a new fdb into hash with only addr set. This is
not good, since there are callers, that search the hash w/o the lock
and access all the other its fields.

This is due to the missing include of linux/usb/composite.h - this patch
adds the missing include.
In addition there's also a problem in fsg_common_init() where we memset
'common', but we use the size of a pointer to 'struct fsg_common' as the
size argument to memset(), not the actual size of the struct. This patch
fixes the sizeof so we zero the entire struct as intended.

CRED: Fix kernel panic upon security_file_alloc() failure.
In get_empty_filp() since 2.6.29, file_free(f) is called with f->f_cred == NULL
when security_file_alloc() returned an error. As a result, kernel will panic()
due to put_cred(NULL) call within RCU callback.
Fix this bug by assigning f->f_cred before calling security_file_alloc().

drm: Only set DPMS ON when actually configuring a mode
In drm_crtc_helper_set_config, instead of always forcing all outputs
to DRM_MODE_DPMS_ON, only set them if the CRTC is actually getting a
mode set, as any mode set will turn all outputs on.

deb-pkg: Fix building outside of source tree (O=...).
When building linux-headers package using deb-pkg, builddeb erroneously assume
current directory is the source tree. This is not true if building in another
directory, using make O=... deb-pkg.
This patch fix this problem.

r8169: RxFIFO overflow oddities with 8168 chipsets.
Some experiment-based action to prevent my 8168 chipsets locking-up hard
in the irq handler under load (pktgen ~1Mpps). Apparently a reset is not
always mandatory (is it at all ?).

Missed ~55% packets. Note:
- this is an old SiS 965L motherboard
- the 8168 chipset emits (lots of) control frames towards the sender
- RTL_GIGA_MAC_VER_26
The chipset does not go into a frenzy of mac control pause when it
crashes yet but it can still be crashed. It needs more work.

r8169: use RxFIFO overflow workaround for 8168c chipset.
I found that one of the 8168c chipsets (concretely XID 1c4000c0) starts
generating RxFIFO overflow errors. The result is an infinite loop in
interrupt handler as the RxFIFOOver is handled only for ...MAC_VER_11.
With the workaround everything goes fine.

lockdep, timer: Fix del_timer_sync() annotation
Calling local_bh_enable() will want to actually start processing
softirqs, which isn't a good idea since this can get called with IRQs
disabled.
Cure this by using _local_bh_enable() which doesn't start processing
softirqs, and use raw_local_irq_save() to avoid any softirqs from
happening without letting lockdep think IRQs are in fact disabled.

but they are not being followed. This patch updates them to reflect
the requirements for THERMAL.
This build error is caused by STUB_POULSBO selecting ACPI_VIDEO
when ACPI_VIDEO's config requirements are not met.

net: Fix bug in compat SIOCGETSGCNT handling.
Commit 709b46e8d90badda1898caea50483c12af178e96 ("net: Add compat
ioctl support for the ipv4 multicast ioctl SIOCGETSGCNT") added the
correct plumbing to handle SIOCGETSGCNT properly.
However, whilst definiting a proper "struct compat_sioc_sg_req" it
isn't actually used in ipmr_compat_ioctl().
Correct this oversight.

USB: fix race between root-hub resume and wakeup requests
The USB core keeps track of pending resume requests for root hubs, in
order to resolve races between wakeup requests and suspends. However
the code that does this is subject to another race (between wakeup
requests and resumes) because the WAKEUP_PENDING flag is cleared
before the resume occurs, leaving a window in which another wakeup
request might arrive.
This patch (as1447) fixes the problem by clearing the WAKEUP_PENDING
flag after the resume instead of before it.
This fixes Bugzilla #24952.

USB: prevent buggy hubs from crashing the USB stack
If anyone comes across a high-speed hub that (by mistake or by design)
claims to have no Transaction Translators, plugging a full- or
low-speed device into it will cause the USB stack to crash. This
patch (as1446) prevents the problem by ignoring such devices, since
the kernel has no way to communicate with them.
